PRODUCT SERVICE CODE (PSC)/NAICS INFORMATION: Product Service Code: 6515 (Medical and Surgical Supplies) NAICS Code: 325413 (In-Vitro Diagnostic Substance Manufacturing) DESCRIPTION OF REQUIREMENT DEXCOM Glucose Sensors (2 Line items) for: Department of Veteran Affairs Leavenworth CMOP - 760 5000 S. 13th Street Leavenworth, KS 66048 A general listing is notated below: Line Item 1 SENSOR, GLUCOSE, CONTINUOUS DEXCOM G4, QTY 400 Line Item 2 SENSOR, GLUCOSE, DEXCOM G6, QTY 400 All offers will be evaluated by the pharmacy for technical acceptability, please include all information requested on Request for Quotes. SOLICITATION: All responsible open market sources may submit a quotation, which if timely received by offer due date/time specified on Standard Form (SF) 1449 of solicitation, shall be considered by this agency. All offers will be evaluated by the pharmacy for technical acceptability. Solicitation 36C77019Q0640 will be posted at www.fbo.gov approximately July 31,2019 or shortly thereafter. All offers must be received by closing Thursday, August 1, 2019 at 3:00pm CST. Please direct any questions in writing to Karen R.
