- THIS IS A SOURCES SOUGHT ONLY. It is NOT a Request for Proposal, a Request for Quotation, an Invitation for Bid, a Solicitation or an indication that Network Contracting Office 22 (NCO 22 Gilbert) will contract for the items contained in the Sources Sought Notice. This Sources Sought Notice is part of a Government market research effort to determine the scope of industry capabilities and interest, in accordance with Federal Acquisition Regulation (FAR) Part 15.201, response to this Sources Sought Notice are not offers and cannot be accepted by the Government to form a binding contract. Responses to this Sources Sought Notice are strictly voluntary and are considered as information only. The Government will not pay respondents for information provided in response to this Sources Sought Notice. Responses to this Sources Sought Notice will not be returned and respondents will not be notified of the result of the review. This Sources Sought Notice does not restrict the Government's acquisition on a future solicitation. The purpose of this notice is to gain knowledge of potential qualified sources capable of providing the following: The contractor shall furnish each Veteran referred for care under a tentative contract with the following basic services: Treatment Planning Objective: The contractor to provide services of its Community Based Health Care for Homeless Veterans (HCHV) program. The goal of the HCHV program is to remove the homeless Veterans from the streets or habitation that is unfit for Veterans and place them in a community-base, residential environments with sufficient therapeutic services to meet the needs of those Veterans. The need for a homeless transitional program that provides shelter for the Severely Mentally Ill (SMI) disabled and aged Veterans is necessary. The VA does not have the capability to provide these types of facilities and service inhouse at the present time nor does it anticipate the ability to do so in the near future. Therefore, there is a current need in place to prevent interruption of services. Requirement The requirement is for transitional housing for Veterans with serious mental illness and/or homeless. Transitional housing is intended to provide short term case management services as a step up or step down housing program. Veterans either are admitted directly off the streets or they are discharged from a pending in-patient or residential care. Customary practices Contract type: Firm Fixed Price hourly rate IDIQ contract Commercial purchase payment terms. The Phoenix VA Health Care System (PVAHCS) will process payments for services rendered. Facility Location: Must be located in the Phoenix metropolitan area. Acceptance: Government Risks: This requirement is for commercial services therefore establishment of a firm fixed price contract involves minimal risk. The requirements of any laws and regulations unique to the services The department of Veterans Affairs has established Health Care for Homeless Veterans (HCHV), in accordance with 38 USC 1720A and Public Law 101-237. This requirement is also in accordance with 38 USC 2031 and Public Law 114-58. The NAICS code for this procurement is 624229 (Other Community Housing Services). Price Schedule: The price schedule shall be a firm-fixed-priced hourly rate for each period of performance (POP).
