- VA Network Contracting Office 23, Black Hills, is issuing this Sources Sought Synopsis to determine the suitability for a Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), or other Small Business set-aside for Regulated Medical Waste Removal services for the Black Hills Health Care System, SD area (see Statement of Work below). This is not a request for quotes or proposals and any such submitted in response to this notice will not be evaluated. Vendors who are interested in this requirement should respond to this notice by providing: a. A short capability statement outlining their ability to provide the service b. A list of other contracts for similar service that they have been prime or a sub-contractor on c. Certification of their SDVOSB/VOSB status through VetBiz.gov (if applicable) d. Company DUNS number e. Company Contact information Annual workload estimate for this requirement is: Description of service Qty Medical Waste Disposal - Weekly Pickup 52 Respond directly via email at: Stephen.holly@va.gov Statement of Work GENERAL INTENTION: The purpose of this solicitation is to provide pick up, treatment, and disposal of regulated medical waste at a minimum of at least one time per week at three sites for the Veterans Affairs, Black Hills Health Care System located at Hot Springs, Fort Meade, and Rapid City, South Dakota, hereinafter referred to as VA. LOCATIONS: VA Black Hills Health Care System; 113 Comanche Road Fort Meade, SD 57741 500 North 5th Street Hot Springs, SD 57747 3625 5th Street Rapid City, SD 57701 GENERAL REQUIREMENTS: Contractor shall provide at a minimum, 40-gallon red reusable Rubbermaid containers, or equal type. At least twenty-four (24) each at Hot Springs site, at least thirty-six (36) each at the Fort Meade site, and at least one (1) at the Rapid City site. Contractor shall provide all containers, bags and required packaging material necessary to perform this contract. Contractor s collection containers shall be odor free and must be fully enclosed, leak proof, fire retardant and shall be cleaned and sanitized with a hospital grade EPA registered disinfectant prior to drop off at each individual site. Contractor shall comply with all appropriate VHA Directives; Occupational Health and Safety Administration; U.S. Department of Transportation (USDOT); U.S. Environmental Protection Agency (USEPA); and State of South Dakota Regulated Medical Waste (RMW) regulations as they pertain to the packaging, transportation and treatment of regulated medical waste. The Regulated Medical Waste Contractor shall also provide evidence of required USDOT, USEPA, and /or State of South Dakota RMW training and disposal. This includes, but not limited to, transportation, labeling, treatment, and disposal of regulated medical waste. Off-site transportation and treatment of regulated medical waste shall be by an approved treatment technology and follow all State and Federal Laws. Proposal package shall include the location and method of treatment, as well as, the necessary permits and/or licenses to conduct this type of business. Any changes during the contract shall be approved by the Contracting Officer. Contractor shall accept all responsibility for any required reporting to the appropriate regulatory agency, the Contracting Officer, and the Contracting Officer Representative and clean-up of any spillage that may occur by Contractor personnel after Contractor leaves the storage sites at any of the VA s facilities. Spill cleanup shall meet regulatory compliance guidelines. Spillage at the storage sites shall be reported to the appropriate Contracting Officer s Representative or Point of Contact and shall be cleaned up by the VA personnel. Contractor shall submit copies of all appropriate permits/licenses along with proposal package. The Contracting Officer shall maintain copies of the permits/licenses. The Contractor shall assure that the Contracting Officer has copies of all current permits/licenses. Any changes shall immediately be brought to the Contracting Officer s attention and all permits/licenses for treatment/transportation, etc., shall be kept current at all times. This requirement includes the certificate of insurance, as well as, the state issued permits/licenses. Contractor shall provide a copy of the medical waste manifest at the time of pick up to allow for tracking of regulated medical waste. When invoices are submitted for payment, a second copy of the completed manifest with appropriate signatures will have a minimum of the following information indicating; the facility, the date of pick up, the date of destruction, the number of containers, the weight of the containers, shall be submitted to the Contracting Officer Representative. At a minimum, this information is to be submitted monthly. All manifests and invoices shall be provided to the Fort Meade VA Facility. Contractor shall transport all regulated medical waste from each VA facility directly to the designated treatment facility. Storage of regulated medical waste shall not be allowed. Contractor shall dispose of refuse in such a manner as to not cause conditions detrimental to public health or to constitute a public nuisance. Contract shall assume full responsibility for compliance with all federal, state, city, and county laws that govern the rules and regulations for the removal of waste and medical waste materials. The VA shall insure that containers are filled only with materials conforming to the description of regulated medical waste as described by VHA Directives, OSHA, Federal, State, and local laws. CONTRACTOR VEHICLES: Contractor shall use vehicles that do not permit loss of regulated medical waste. Open box trucks, with canvas covers, shall not be permitted. Collection vehicles shall be maintained in good repair, operate safely, relatively clean, and not be considered unsightly. Vehicles considered by the COR not to meet the prior minimum standards will not be permitted to collect regulated medical waste on VA property until corrected. The Contractor shall have sufficient vehicles for backup of regularly assigned vehicles to insure completion of contract requirements in the vent of breakdowns or other unforeseen problems. SAFETY AND SECURITY PRECAUTIONS: Contractor shall take all required precautions regarding fire and safety policies/procedures within VA. The contractor is responsible for immediately notifying the COR of any existing or potentially unsafe conditions noted or with possibility to develop in any space the contractor is aware of i.e., frayed electrical cord, dripping or seeping of fluids. Contractor shall comply with all Occupational Safety and Health Act (OSHA) and applicable laws/regulations for the protection of his/her employees. The Government shall assume no liability or responsibility for the Contractor s compliance or noncompliance with such requirements. EVALUATION CRITERIA: Submitted bids will contain all requested information above. Bidding contractors must provide adequate documentation and explanations to meet all the requirements spelled out in the Performance Work Statement above. Requirement 1. Provide copies all required permits. Requirement 2. Provide a description of the disposal site and method of neutralization of the materials. Requirement 3. Provide a sample of the manifest to be used that will provide all required information. Requirement 4. Provide a sample pick up schedule Requirement 5. Provide a minimum of three references of similar service provided to other customers. Requirement 6. Provide five-year pricing structure. CONTRACTOR STAFF: All staff provided by the Contractor will be clean, well groomed, and provide an identifiable uniform that includes a company logo. All contractor staff will be courteous and not engage in any of the following; using profane or degrading language, using any offensive body language, telling any unacceptable jokes or stories, be argumentative, fighting or horseplay, etc. Any contractor staff engaging in any of the above behavior will be asked to leave the facility immediately. PRICING: Contractor pricing should be based on either, per pound, per barrel, or per pick-up of the three locations identified above.
