- This is a sources sought notice 36C24419Q0924; this is neither a solicitation nor a promise of future work. Wilkes-Barre VAMC, (693) is seeking SDVOSB/VOSB and any SBC regardless of Socio-economic category that can provide employee uniforms. Vendors expressing interest must meet the requirement cited in the Statement of Work below. The veteran business must be registered in SAM and certified in VETBIZ (VIP). The small businesses must be registered in SAM. All Vendors interested in, and with the capability and capacity to fulfill this requirement, are required to furnish the following minimum information in reply to this sources sought: Company Name, address, Government Business Point of Contact, telephone number and email address. Company DUNS number, CAGE Code, and Tax ID numbers. Identify any current Federal Supply Schedule, GSA Contracts held, if none then state that no FSS/GSA schedules are held. Responses shall be submitted electronically, via email to Donna Toliver at donna.toliver3@va.gov due no later Monday 07/22/2019 at 1:00 pm. STATEMENT OF WORK (SOW) Purpose It is the intent of this request to establish a firm fixed price, base plus four option period contract with a qualified firm to provide staff uniforms for the Wilkes-Barre VA Medical Center. Service is required for the VA located at 1111 East End Blvd., Wilkes-Barre, PA 18711, II. Statement of Needs/Scope of Work A. Measurement: The successful Contractor(s) shall arrange to measure all employees within two (2) weeks of award. One (1) week prior to the time of measurement, the Contractor will provide examples of the various uniform style options listed herein for the respective departments/divisions. VA staff will be fitted with care to insure proper sizing and professional appearance. Any shrinking factors should be considered when sizing. B. Increase/Decrease in Size: In the event of employee size changes due to increase or decrease in weight or height, the Contractor will make the necessary size adjustments and/or provide new uniforms at no cost to VA. C. Extra Large Sizes: There is to be no additional charge for extra large sizes. D. Long Sizes: Each item must be available in long (tall) sizes. E. Condition: All uniforms must be new and unused at the start of the contract. Like-new uniforms are not acceptable. The Contractor must maintain the uniforms in an acceptable condition throughout the term of the contract. Determination of acceptable condition of the uniforms rests solely with the VA contract administrator and will be unconditionally accepted by the Contractor. The appearance of the uniforms reflects on VA and the Contractor. Uniforms shall be clean, pressed, well mended, and professionally altered (hemming, etc.) when necessary at no additional cost to VA. Uniforms determined not to be in good condition by the contract administrator or department contact will be returned to the Contractor and will be removed from the invoice until replaced or altered to an acceptable condition. F. Repairs: Repair tags shall be provided at the beginning of the contract and as needed throughout the contract. Any uniform tagged for repairs will be returned to VA in a repaired condition or will be replaced within two (2) weeks at no cost to VA. Repairing uniforms with patches will not be acceptable. Uniforms delivered with rips, holes, missing buttons, etc. will be sent back and no rental charge will be paid for that item until it is returned in an acceptable condition. G. Uniform Life: The average life of a uniform is approximately two (2) years. All uniforms are to be replaced when the condition does not meet the acceptable condition criteria. H. Uniforms Not in Use: When employees are not active due to extended illness, workman s compensation, etc., VA will notify Contractor and there will be cessation of charges for that employee s uniforms. VA will notify Contractor when delivery is to continue for that employee. I. Environmental/Manufacturing: The VA will not be responsible for environmental / manufacturing charges. J. Inventory: Each person receiving clothing will have 5 pairs of uniforms. Initial delivery of uniforms will consist of the complete order issue as described herein. K. New Orders/Change Orders: Requests for new staff uniforms/items, size changes, lost or destroyed uniforms, stained uniforms, or other situations requiring a new issue of uniforms shall be filled within two (2) weeks of notification by the VA. L. Samples: Contractor shall provide VA with uniform shirts and pants in each available size to be used for measuring new employees at no cost the VA. Employee individual uniform sets will consist of five (5) pants and five (5) short sleeve shirts. Supervisors are to get (2) pants and (2) shirts. Delivery: Delivery is to be made once per week during normal business hours 8:00 a.m. to 3:30 p.m. M. Environmental Services: Uniforms will be required for 72 employees and both male and female. 1. Shirts, Short Sleeved: Pressed, hanger hung, short sleeve, 65% polyester / 35% cotton, double breast pocket. Color choice is black. 2. Pants: Pressed, creased, hanger hung, 65% polyester / 35% cotton, zippered, without pleats and with belt loops. Color choice must be khaki. 3. Cargo Pants: Pressed, creased, hanger hung, 65% polyester / 35% cotton, zippered, cargo style, without pleats and with belt loops. Color choice must be khaki. 4.
