Description

Scope of Work Project Title: Renovate C3/D3 Wards (SA) Project Number: 630A4-14-436 Scope of Work: The scope of this project is an extensive renovation of the 3rd Floor of Buildings 85 & 86 known as the CLC C3D3 Unit. The project includes the renovation and upgrade of the Architectural, Mechanical, Electrical and Plumbing Design of the CLC Unit. The existing space is approximately 15,603 square feet, located on the 3rd Floor of Buildings 85 & 86 of VA Medical Center, St Albans Campus, Jamaica, NY. The work consists of but is not limited to the renovation of semi-private CLC beds wards to a portion of single and double occupancy patient rooms. Work also consists of, but is not limited to, the renovation and construction of additional areas and rooms required for the CLC functions including: Kitchenette, activity spaces, supply storage, nurses work areas, reception and offices, and accessible toilets/showers as related to the patient rooms. Work shall also include new and upgraded MEP, control, and communication systems, as shown in the construction documents and specifications, including but not limited to the following systems: air distribution and ventilation, plumbing, electrical, telephone, data, TV, nurse call, medical gases and building management. Asbestos remediation shall be required. Upgrades will include additional needs for emergency power, the generation of which will be provided via another project. New and upgraded air distribution and ventilation system, plumbing systems, electrical systems, asbestos remediation are included for the entire 3rd floor of Bldgs 85 & 86 (C3 D3). Minor renovation for the distribution of MEP systems thru the lower two floors is also included. All work described above, for this phase of the project are shown and described in the construction documents and specifications. Contractor shall be required to provide the following documentation for all of his/her personnel working on VA property: VA issued identification badges OHSA Training certifications Construction Safety and Accident Prevention plans TB tuberculin test certifications All contractor employees and subcontractor employees requiring access to VA information and VA information systems shall complete the following before being granted access to VA information and its systems: Successfully complete VA Privacy Training for contractors who have no access to PHI or Access to TMS. The contractor shall provide to the COR a copy of the training certificates and certification of signing the Contractor Rules of Behavior for each applicable employee within 1 week of the initiation of the contract and annually thereafter, as required.   Failure to complete the mandatory annual training and sign the Rules of Behavior annually, within the timeframe required, is grounds for suspension or termination of all physical or electronic access privileges and removal from work on the contract until such time as the training and documents are complete. BASE BID AND ALTERNATES: BASE BID: Work includes general construction, alterations of the third floor of Building 85 and Building 86 at the St. Alban s VAMC. Construction Estimate: $ Completion Time: 700 days DEDUCT ALTERNATE #1: BASE BID minus the deletion of all interior walls, floor finishes, ceilings, and plumbing fixtures for rooms C308 through C318A and C323. Construction Estimate: $ Completion Time: 600 days DEDUCT ALTERNATE #2: DEDUCT ALTERNATE #1 minus the deletion of all interior walls, floor finishes, ceilings, and plumbing fixtures for rooms C302 through C307A, C319, C319A, and C324.
