Description

The Department of Veterans Affairs in El Paso Veterans Affairs Health Care System (ELPVAHCS), TX is posting this combined Synopsis/Solicitation for AUTOMATED AUTO-REFRACTOR/KERATOMETER through the Contracting Officer in San Antonio, Texas. The government intends to make a contract award to the Lowest Price Technically Acceptable vendor that can provide the required items in accordance to the attached documents, as a result of this solicitation for quotes. This action is a total small business set-aside for service-disabled veteran owned small business (SDVOSB) only. The NAICS is 339115. Responses to this notice will be used by the Government to make appropriate evaluations and award decisions. GENERAL The contractor shall comply with Federal, State, local laws and regulations applicable to the performance of this contract. The El Paso VA Health Care System (EPVAHCS), Eye Clinic, requires the purchase of AUTOMATED AUTO-REFRACTOR/KERATOMETER. Delivery, installation and training to be completed by sixty (60) calendar days date of award. GENERAL REQUIREMENTS The contractor shall provide equipment suitable to provide the EPVAHCS Eye Clinic Section with the ability to accurately and efficiently utilize the equipment to successfully obtain automated refraction results and corneal curvature measurements to use toward completion of the standard ophthalmic and optometric evaluation. The automated refractor is a computer-controlled machine used during an eye examination to provide an objective measurement of a person's refractive error and prescription for glasses or contact lenses. This is achieved by the system by measuring how light is changed as it enters a person's eye. The contractor shall provide shipping and handling, all tools and equipment, technical information, travel, installation, set up and training on use of the unit. 2.3. The Contractor is expected to respond with 24 hours of the maintenance call. No reports or documentation is required to be submitted after repairs. 2.4. EQUIPMENT REQUIREMENTS a. Salient characteristics, technical specifications and imaging specifications the shall be met for consideration. MUST INCLUDE ALL THE FOLLOWING for consideration: see below AUTOMATED AUTO-REFRACTOR/KERATOMETER Must have fully automated operation Must have easy-to-use color touch screen/panel Color Touch screen must contain Finger Touch selectable tabs/controls for the following: ID R/L eye indication Chinrest up/down Position reset Measuring mode Cataract Fixation target Target image Print All clear Corneal diameter Auto/manual Move head forward/backward Setting Must not have any Joystick control SHALL HAVE 360 DEGREE ROTABLE MONITOR Shall have a flexible layout and space saving design Shall ensure fast, accurate, and repeatable refraction and keratometry measurements Right/Left eye automated measurement Must have and use Auto Focus capability Must be able to simply touch the pupil on the color monitor and the unit must automatically obtain Keratometer/Refraction results immediately Must allow for all features and procedures to be fully controlled and started from the color touch screen monitor Must have manual mode setting as well, controlled thru the color touch screen when activated Must have compact design, with depth of 449mm Must take corneal measurements of peripheral corneal radius Must automatically obtain inner KRT measurements to be used with contact lens measurements Must have Spherical refractive power measurements of -25 d to +22 D, in increments of either 0.25 D or 0.12 D steps Must have cylindrical refractive power measurements of 0 D to +/- 10 D, in increments of either 0.25 d or 0.12 D steps Must have astigmatic axial angle: 0-180 degrees, in 1 degree to 5-degree steps Shall have corneal curvature measurement radius of 5mm to 10mm (in 0.01mm steps) Shall have corneal refractive power 67.50 D to 33.75 D with a corneal refractive index of 1.3375. Must have PD measurement range of 20mm to 85mm (in 1mm steps) Dimensions must match the following: 286~326mm (W) x 445~526mm (D) x 466~-615mm (H) 3. DELIVERY AND INSTALLATION 3.1. Equipment shall be delivered to the main facility below: EPVAHCS Eye Clinic Section, 2nd floor Main Building 5001 N. Piedras El Paso, TX 79930 3.2. The contractor shall provide Delivery, installation, start-up assistance, operator instructions, training on the use of the system for eye clinic personnel. 3.3. Installation will be completed within SIXTY (60) calendar days after date of award. 3.4. The contractor shall provide TWO (2) hard-copy (paper) Operator s Manuals. One for eye clinic department and one for biomed staff. 4. WORK HOURS: 4.1. Normal Work Hours: The service schedule shall be developed between the contractor and Contractor s Representative (COR) prior to any service being performed. 4.2. The following is a list of U.S. Government holidays. If the holiday falls on a Saturday, the proceeding Friday is observed as the holiday; if the holiday falls on a Sunday, the following Monday is observed as the holiday and any other day specifically declared by the President of the United States to be a national holiday. PERSONNEL 5.1. The contractor shall provide in writing the name and phone number of personnel within (10) ten calendar days of the award of the contract. The personnel shall be a qualified and experienced to oversee the personnel assigned to perform the installation and maintenance services. The contractor shall correspond with the COR on a regular basis to discuss any problems that the contractor or contractor s personnel may be experiencing during the performance of this contract. Unresolved problems shall be referred to the Contracting Officer for resolution. 5.2. Contractor Service Personnel (CSP). All subcontractors performing work for primary contractor shall meet all specifications and standards that apply to CSP under this agreement. CSP shall maintain clean and neat appearance and shall wear an identification badge at all times when performing services at the Government site. Identification badges shall be worn in a clearly visible area of the outer garment. The COR shall furnish this badge. Due to conflict of interest, the contractor shall not employ a current DOD employee, military or civilian to provide services under this contract. 5.3. Government point of contact (POC). The COR shall be the Government s POC. If required, the COR shall be designated in writing to the Contractor and the scope of authority shall be set forth therein. Contractor shall respond only to calls from COR or a designated representative from the Medical Center. 5.4. SECURITY STATEMENT: The Vendor shall not transfer any VA information to a location outside the VA and only to VA locations determined by the VA System Administrator.   The information in these systems may be covered by the Privacy Act 1974 which contains criminal penalties of abuse of information. The Vendor and all VA employees are required to immediately report any security violations to the Information Security Officer.  No other security statements are required. 6. ACRONYMS AND DEFINITIONS 6.1. Contracting Officer (CO). A person duly appointed with the authority to enter into and administer contracts on behalf of the U.S. Government. 6.2. Contracting Officer s Representative (COR). An individual designated in writing by the Contracting Officer to act as an authorized representative of the Contracting Officer to perform specific contract administrative functions within the scope and limitations as defined by the Contracting Officer. 7. Equipment Ownership. 7.1. Title to equipment shall remain with the contractor until installed and established. After completion, a satisfactory inventory and inspection is completed by Contractor, COR and Maintenance personnel. Upon approved inspection, title, equipment, accessories and ownership shall be released to EL Paso Veterans Affairs Health Care System (ELPVAHCS). 8. Limited Warranty. 8.1. All equipment listed to the attached quote, shall be fit and sufficient for the purpose intended as set forth in the user manuals; and merchantable, of good quality and free from defects in materials or workmanship; for a period of one (1) year from the date of the first invoice under this agreement. The contractor shall provide a minimum of one (1) year warranty. The warranty will begin upon date of installation.
