Description

This is not a solicitation announcement. This is a Sources Sought Synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (HubZone, 8(a), small, small disadvantage, veteran owned, women owned or service disabled veteran owned small business) relative to NAICS code 621420 (size standard $15 Million) for the procurement of Alcohol & Substance Abuse Counselor for a Base Year plus 4 Option Years in Moapa, Nevada. The contractor shall provide counseling services for alcohol and substance abuse. Functions as a member of a multi-disciplinary treatment team at the Moapa Clinic in Moapa Nevada. Responsible for integrating all client information and initially formulating treatment plans for assigned clients. He/she is the primary therapist and case manager; services during treatment include (but not limited to) assessment, treatment planning, individual and family counseling, recommending aftercare plan. Contractor records clients' progress in the medical record using notations and format designated in the policies and procedures. Entries are made on completion of assessment, upon implementation of treatment plan, clients' progress and changes in client's condition and any changes in treatment plan as well as after any significant therapeutic contact with the client. Documentation will be stored in client record at the clinic and in EHR when that is available. Contractor will comply with the Standards of Conduct for Federal Employees which deals with courtesy to the external and internal customers.
