- BACKGROUND: VA Health Care Upstate New York is seeking bi-monthly (24 service calls per year) Pest Control Services at the VA Community-Based Syracuse VA Medical Center and monthly (12 service calls per year) Pest Control Services at the Rome, NY Outpatient Clinic (CBOC). PLACE OF PERFORMANCE: Syracuse VA Medical Center, 800 Irving Ave, Syracuse, NY 13210 Building No.1 - Main Hospital Building No.2 - Administrative Offices D Wing Research Laboratory Building No.1 Trash Compactor and Dumpster Building No.16 Animal Research facility Building No. 19 Parking Garage Spinal Cord Injury Addition (SCI Bldg.) Rome Outpatient Clinic (CBOC), Griffiss Business Park, 125 Brookley Road, Rome, NY 13441-4301 Building 510 (Main Clinic) Garage. SCOPE: The contractor shall provide all labor, supervision, supplies, materials, equipment and transportation to develop and implement an integrated pest management (IPM) plan for the interior and exterior of the Syracuse VA Medical Center and the Rome, NY Community-Based Outpatient Clinic (CBOC). All work is to be performed by qualified/certified applicators in accordance with all federal, state and local requirements and regulations. All services shall be required to be done in a manner that ensures the health and general well-being of patients, staff, and visitors. All infestations will be addressed until complete elimination of pests. All services shall be provided during normal working hours from 8:00 AM and 4:30 PM, Monday through Friday except Federal Holidays and as noted otherwise. Note: Canteen Services shall be serviced after 3:00 PM. SITE VISIT: If it is determined that a Site visit is required, it will be scheduled by the Contracting Officer. SPECIFICATIONS: The contractor shall conform to all federal, state, and local regulations governing examining and licensing of pest control operators, performance of pest control, use of approved pest control chemicals and equipment. Contractor shall utilize proper disposal methods for all products/containers which may be in effect for the area where the work under the contract will be performed. Contractor shall provide an Integrated Pest Management (IPM) plan which the contractor will execute in performance of this requirement. The IPM requires approval by the COR prior to implementation. The contractor s IPM plan shall include but is not limited to the following: Inspect to determine pest management measures are appropriate and required. Recommend and communicate to the COR environmental sanitation practices that restrict or eliminate food, water or harborage for pests and recommended engineering practices that limit entry of pests. Selection and utilization of non-chemical control methods, which eliminate, exclude or repel pests, i.e. insect electrocution devices, traps, caulking, air screens, etc. Collection and disposal of all pests. Selection and use of the most environmentally sound pesticide(s) to affect control when chemical control methods are necessary. Control general structural arthropod pests (i.e., cockroaches, ants, carpet beetles, spiders, carpenter ants, carpenter bees, etc.). Control flying insect pests (i.e., housefly, stable fly, blow flies, etc.). Control predatory pests (i.e., lice, bedbugs, fleas, mites, ticks, bees, wasps, mosquitoes, scorpions, ect.). Control stored product pests (i.e., saw tooth grain beetle, red confused flour beetle, frog, beetle, grain moths etc.). Control mice and rats (i.e., house mouse, field mouse, roof rat, Norway rats, etc.) Control pest birds (i.e., pigeons, sparrows, blackbirds, etc.) Control other vertebrate pests (i.e., dogs, cats, bats, squirrels, gophers, moles, skunks, snakes, rabbits, raccoons, etc.) Control wood destroying organisms (i.e., subterranean termites, dry wood termites, fungi, wood boring beetles etc.). Control aquatic pests (i.e., mosquito larva/pupae, algae etc.). SERVICES TO BE PERFORMED: Contractor shall inspect Outpatient Clinic once per month. A detailed service report shall be submitted to the Environmental Management Services Department. Inspected areas to include employee locker and break rooms, janitorial closets, laundry rooms, food service areas (kitchens, storerooms), Canteen and retail store, vending machine areas, food service carts, patient wards, exam rooms, intensive care wards, surgical suites, dialysis rooms, autopsy rooms, floor drains, sinks, and trash dumpsters. Perimeter Protection Exterior: Contractor shall implement measures to intercept and control ants before they enter the facility and install and maintain exterior ant bait stations during the ant season. To prevent or control other crawling insects, the contractor shall perform the necessary granular and residual insecticide applications during regular monthly service on an as need basis. Interior: To control ant infesting inside, the contractor shall discretely apply ant gel bait into cracks and crevices near nest sites and ant trails. All ant bait stations shall be locked and secured, and an ant bait station diagram shall be included in the service log book. Only the minimal amount of insecticide required shall be used and all materials must be EPA approved and applied according to the label, laws and regulations. Checkpoint Rodent Program Exterior: Contractor shall intercept and control rodents before they enter the facility. The contractor shall install and maintain enough bait stations and shall document placement on an equipment diagram. The contractor shall service all bait stations per visit. Interior: The contractor shall place and maintain enough interior mechanical traps (non-snap trap) and shall document placement on a rodent equipment diagram. The contractor shall service these traps per visit. A rodent diagram shall be included in the service log book. All bait stations shall be locked and secure. All traps and bait stations shall be dated during each service visit and log sheets must note the conditions found during the time of service. 3. Cockroach Elimination Program: If cockroaches are present, the contractor shall discretely apply the necessary bait, residual, contact, and dust insecticides into cracks and crevices and voids where cockroaches may hide. Residual, dust, or contact insecticide treatments shall be limited to application within food service, break rooms, laundry, and common areas. If cockroaches are present in sensitive areas such as patient rooms and medical units, bait must be placed out of the sight of staff members and patients. All baits must be odorless, non-flammable, non-corrosive, have no inhalation hazard and must be classified as category IV pesticides, the lowest EPA toxicity rating. Only the minimal amount of insecticide required shall be used and all materials shall be EPA approved and applied according to the label, laws, and regulations. All insecticide applications shall be recorded in the customer service report provided by the contractor. Bed Bug Program: The contractor shall inspect both the Medical Center and Rome Outpatient Clinic for Bed Bugs, and if warranted, will provide an integrated Bed Bug service that includes the use of monitors, baits, and / or the use of liquid exterminator. Contractor is required to respond to the VA within one hour of a call for Bed Bugs. Contractor must provide an estimated time of arrival not to exceed one business day from the time of call. In case that Bed Bugs infiltrate facility, the contractor and the Government COR will coordinate to develop a removal plan. Drain Program: Contractor shall provide two (2) cases per month of BIOSYS, Cleaning Agent. Man Contractor shall provide two (2) cases per month of BIOSYS, Cleaning Agent. Manufacturer: Biological System, Inc. Cleaning agent is used in kitchen areas for drains to keep Fruit Flies at bay. CALL BACKS: The contractor shall within 12 hours after receipt of all other non-bedbug Emergency call notification from the Contracting Officer or his/her designee, perform Integrated Pest Management (IPM) Services to correct the emergent condition. In Non-Emergency calls, the contractor shall report within 24 hours after receipt of notification by the COR for non-scheduled services. REPORTING FOR SCHEDULE SERVICES: The contractor shall report to Room AO5 in building 2 on the ground floor for the Syracuse VA Medical Center and Facility Management Office, Ground Floor, for the Rome Outpatient Clinic, on scheduled days for any instructions or to review the Pest Report Log for location of infestations, as called in by wards, clinics, etc. They will submit a written report of all activities following each visit, with the signature of the COR or designee responsible. SAFETY FACTOR: All rodenticides or traps shall be placed only at times and in the areas approved by the COR or his/her designee. Rodenticides, and/or traps shall be replaced as necessary and per manufacturer recommendations. No pest control material or trap shall be placed where it may be recovered by patients, nor shall such material be allowed to contact food or cooking utensils. STORAGE OF PEST CONTROL MATERIALS: Storage of pest control materials or equipment are prohibited without the written approval of the COR. INTERFERENCE TO NORMAL FUNCTION: The contractor may be required to interrupt their work at any time so as not to interfere with the normal functioning of the facility, including utility services, fire protection systems, and passage of facility patients, personnel, equipment and carts. In the event of an emergency, contractor services may be stopped and rescheduled at no additional cost to the government. Contractor personnel shall inform the COR or the designee of the need to gain access to secured areas. If access is required to secure areas, prearranged scheduling will be made with the COR.
