Description
This is a Solicitation for Construction Services prepared in accordance with the format in FAR Part 15, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested. Solicitation number 36C26319R0068 is issued as a Request For Proposals (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-91. The VA anticipates one firm-fixed-price contract award for all resources and labor necessary to add approximately 13,500 square feet by adding a second floor to Building 111 as identified in the SOW herein for the St Cloud VA Health Care System. Additional requirements, specifications, and regulations are listed within the attached solicitation documents. A list of the contract line items, quantity, and unit of measure are listed within the attached solicitation document(s). This procurement is being solicited as a Total 100% Set Aside for Service Disabled Veteran Owned Small Businesses. Prospective offerors must be registered and current in the System for Award Management (SAM) by the RFP closing date. www.sam.gov. Any Contractors claiming to be SDVOSB must be registered and certified in the Vendor Information Pages www.vip.vetbiz.gov by the RFP closing date. Proposals will only be accepted from responsible contractors. The NAICS code for this solicitation is 236220 and the small business size standard is $36.5 million. Proposals are due to the Contracting Office by Thursday, June 27th, 2019 @ 12:00pm CST. Proposals are required to be submitted to Lucas Dahlman, Contract Specialist per the requirements identified in the SF 1442 Solicitation prior to the solicitation closing date and time with subject title: Construct/Expand B111 Mental Health Outpatient Clinic. It is the responsibility of the offeror to ensure their proposal is received in full by the Contracting Office before solicitation closing. Areas to complete when submitting your proposals: Refer to the full SF 1442 solicitation for all required submissions. ***There will be a planned site visit for this solicitation; See SF 1442 for time and location*** All questions must be submitted in writing via email to lucas.dahlman@va.gov. Questions must be received by June 10th, 2019 @ 12:00pm CST. PROJECT TITLE: Expand/Construct Outpatient Mental Health Clinic Project 656-341 SCOPE/BASE BID ITEM: This project will add approximately 13,500 square feet by adding a second floor to building 111. This project will mitigate Minnesota Market utilization gaps for select Ambulatory Mental Health programs, including mental health clinic, psychology, and aftercare/screening/outreach programs. Contractor shall furnish all labor, materials, tools and equipment required to expand/construct an outpatient mental health clinic. The contractor shall completely prepare site for building operations, including demolition and removal of existing structures, and furnish labor and materials and perform work for Expand/Construct Outpatient Mental Health Clinic as required by drawings and specifications. Work includes general construction not all inclusive to: abatement, demolition, removal or relocation of existing structures, relocation of existing utilities, alterations, grading, drainage, site utilities, curbs, walks, pavement, storm water management systems, architectural, structural, mechanical and electrical work. The existing Building 111 will be vacated during construction of this project. Adjacent buildings will remain occupied by the VA throughout construction of this project.
