- April 19, 2019 Blue Spader Contractors Inc. 30375 Northwestern Hwy, Suite 101 Farmington Hills, MI 48334 Subject: NOTICE OF AWARD, Project 506-330, Renovate 8th Floor Mental Health, Contract 36C25019C0049 Dear Mr. Collins, Acceptance is hereby made of your offer for Bid Item No. I, Base Bid, in response to solicitation number 36C25019B0817 dated October 25, 2018. Award is for consideration of Nine Million Two Hundred Thirty-Five Thousand Dollars and Zero Cents ($9,235,000.00), in strict accordance with the contract documents. A fully executed copy of the contract is enclosed. The contract allows Three Hundred Sixty-Five (365) calendar days for completion of the project after issuance of the Notice to Proceed. Contract number 36C25019C0049 and Purchase Order (PO) number 506Z90001 have been assigned to this contract. Please reference the contract number on all correspondence. The PO number must be on all payment applications. You are required to furnish Payment (Standard Form 25A) and Performance Bonds (Standard Form 25) in a penal sum equivalent to 100% of the contract award amount. In accordance with your accepted offer, the bonds must be executed and returned within ten (10) calendar days after notification of award. Only the original bonds are required to be submitted no copies are required. The bonding company used must be listed in the Department of Treasury Surety Circular 570. In addition, a current insurance certificate showing evidence of workers compensation, general liability, and automotive liability must be furnished prior to start of work in accordance with contract clause FAR 52.228-5 Insurance Work on Government Installations. Upon receipt and approval of the bonds a preconstruction meeting will be scheduled with your company and the Notice to Proceed issued.
