- This is a Sources Sought Notice (a) The Government does not intend to award a contract on the basis of this Sources Sought or to otherwise pay for the information solicited. (b) Although "proposal," "offeror," contractor, and "offeror" may be used in this sources sought notice, any response will be treated as information only. It shall not be used as a proposal. (c) Any information received from a contractor in response to this Sources Sought may be used in creating a solicitation. Any information received which is marked with a statement, such as "proprietary" or "confidential," intended to restrict distribution will not be distributed outside of the Government, except as required by law. (d) This Sources Sought is issued by VISN 2 Contracting Office in Bronx, NY for the purpose of collecting information about Specialty Care Bed Service and Preventative Maintenance at the Batavia VA 222 Richmond Avenue, Batavia, NY 14020. The VA is seeking to provide for this requirement as of June 1st, 2019 as a new requirement. The NAICS code identified for this requirement is 811219 (other Electronic and Precision Equipment Repair and Maintenance).The requirement is detailed in the Statement of Work section of this document. (e) Cursory market research has identified several small businesses that may be able to provide for this requirement. Contractors that are able to meet the requirements in the attachment are encouraged to email a capability statement and full information to Evelyn Peguero at Evelyn.Peguero@va.gov. - Part of the purpose of this sources sought is to determine the viability of set aside to a specific socio-economic category. Contractors shall identify the NAICS code for the services being offered as well as their size status under said NAICS. If contractor is a Service Disabled Veteran Owned Small Business (SDVOSB), a Veteran Owned Small Business(VOSB), or any other special category of contractors, then contractor is responsible for providing documentation in support of this. This information will be used to determine the viability of a set aside for this requirement. - Contractors shall also identify any Federal Supply Schedules that may carry the desired services. -Contractors shall identify pertinent point of contact for company, contractor DUNS number for size standard and socioeconomic verification in SAM and VIP. - Contractors shall include any relevant comments about the Attachment(s) if applicable. Statement of Work Specialty Care Bed Service and Preventative Maintenance Background The VA Western New York Healthcare System is currently seeking a firm fixed-price contract to include four (4) option years for parts, labor, travel and all materials needed to perform routine and emergency repairs, and annual preventative maintenance (PM) on eighteen (18) Stryker InTouch beds, eighteen (18) Stryker Isolibrium Support Surfaces, three (3) Joerns Enhanced Bari 10A beds, three (3) Joerns Arise Therapy Mattresses, and fourteen (14) CHG Sprit Select Platform beds located at the VA Medical Center, 3495 Bailey Avenue, Buffalo, New York 14215. Contract must also include parts, labor, travel and all materials needed to perform routine and emergency repairs, and annual preventative maintenance (PM) on four (4) Joerns Enhanced Bari 10A beds, four (4) Joerns Arise Therapy Mattresses, and twenty (20) CHG Spirit Select beds located at VA Medical Center, 222 Richmond Avenue, Batavia, NY 14020. Replacement parts do not include CHG mattresses, batteries, and other disposable or expendable parts. SCOPE OF WORK The contractor must report and sign-in with the COR prior to the starting of work activities. Upon completion of each on-site visit the contractor must debrief the COR on their findings and sign-out of the facility. All work shall be monitored by the COR for completeness per SOW and a formal review by the COR shall satisfy the evaluation of completeness. Provide all mobilization and demobilization services such as setting up a work area including, but not limited to: repair shop, storage area, and office. Area will be provided by the facility. The mobilization work associated with the facilities mentioned shall be done in a safe and workmanlike manner and shall conform to all pertinent state and local laws and regulations. All work shall be performed by Original Equipment Manufacturer (OEM) certified technicians and use new OEM parts. Mattresses needing replacement that are not covered under this contract will be supplied by the respective facility. All repair work performed will be accomplished using the guidelines outlined in the equipment s service manual. Contractor shall comply with all local codes and other applicable codes/requirements. Contractor shall repair deficiencies identified during the PM inspections and/or scheduled ahead of time with the COR. All repair and preventive maintenance cleaning supplies, parts, and materials shall be supplied by the vendor. A stock of common repair parts shall be stored on site. Minor repairs may be made in the patient s room where the situation allows. All preventive maintenance and most bed repairs for VAMC Buffalo shall be done in the bed maintenance room located on the 6th floor in 608 Core. All preventive maintenance and most bed repairs for VAMC Batavia shall be done in a room to be determined by Batavia Engineering. Documentation of all repairs and preventive maintenance is required with each site visit. An annual written evaluation of all equipment status and condition is required. Contractor will be assuming responsibility of an established Annual PM program. In order to remain in compliance, the contractor must adhere to our current schedule of due dates for bed inspections. Locating and retrieval of beds due inspection is the responsibility of the contractor. Annual Preventive Maintenance for Stryker InTouch beds shall consist of the following: Disinfect the exterior of the support surface and remove surface from the bed frame before start of the bed inspection. Inspect all welds for cracks or breaks and ensure all fasteners are secure. Inspect tubing or sheet metal for bends or breaks. Ensure casters are secure, swivel properly and are free of debris. Verify manual and electric brakes apply and release properly. Verify Brake Not Set LED on the footboard and head end siderails illuminates when brakes are not engaged. Verify locking steer caster engages and disengages properly. Verify steer caster latches properly. Ensure Fowler operates properly. Verify litter up/down operates properly from all control points. Ensure bed goes into Trend and Reverse Trend positions from all control points. Inspect IV poles are intact and operating properly. Inspect headboard, footboard, and siderail panels for cracks or splits. Inspect all covers are not damaged and are not producing sharp edges. Verify night light is operating properly. Test CPR release is operating properly from all control points. Verify all siderail motion functions work properly from all control points. Ensure all siderails move, latch, and stow properly. Verify all siderail switches work properly. Verify iBed awareness light bars on footboard and siderails are working properly. Verify all functions on head end siderails work properly, including LEDs. Verify all functions on footboard and headboard work properly, including buttons, touch screen display, and LEDs. Ensure touch screen is accurately calibrated. Perform bed calibration. Verify scale and bed exit system operate properly. Place bed in drive, ensure wheel deploys and Zoom operates properly. Verify motion release switches work properly. Verify Zoom handles are not damaged and function properly. Replace batteries every two years. Inspect batteries for corrosion at the terminals, cracking, expanded or bulging at the sides, or inability to hold full charge, if fails replace batteries. Verify nurse call functions properly from all points, calibrate as needed. Lubricate bed frame where required. Inspect pendant control for operation and any physical damage. Inspect power cord and plug end for frays or damage. Ensure all cables are not worn or pinched. Verify all electrical connections are tight. Verify all ground cables are secure to the bed frame. Test ground impedance is not more than 0.50 Ohms. Test current leakage is not more than 500 microamps. Ensure ground chains are clean, intact, and have at least two links touching the floor. Ensure enclosure is free from wear, tear, stresses, and mechanical damage. Inspect bed for rust or corrosion of parts, replace or repaint as needed. Inspect all labels for legibility, proper adherence, and integrity. Clean and sanitize bed frame. Annual Preventive Maintenance for Stryker Isolibrium Support Surfaces shall consist of the following: Clean and disinfect the exterior of the support surface before start of inspection. Inspect zipper, snaps, and covers (top, bottom, and fire barrier) are free of tears, cuts, holes, or any other damage. If excessive wear is observed on the cover or the fire barrier replace the cover. Ensure support cover labels are legible, have proper adherence, and are free of damage. Inspect mattress handles for rips or cracks. Inspect power cord, integration cable, and plug end are free of damage. Perform a test of the Isolibrium functions. Perform system diagnostics and confirm there are no errors. Inspect internal components for signs of staining from fluid ingress or contamination by fully unzipping the cover. Verify pods are not leaking or cracking. Verify hose connections to the manifold and sensor tubes (both ends) are seated fully. Inspect foam for fluid staining, large tears or large gouges. Verify turn bladders are free of excessive wear and function properly. Ensure low air loss fans freely rotate and are free of debris and dust. Verify Left and Right CPR releases function properly. Ensure both foot box cooling fans rotate freely and are free of debris and dust. Inspect pump spring mount has visible clearance from the foot box bottom. Listen for abnormal pump vibration and noise level. Clean the foot box intake screens. Clean the mesh on the bottom cover. Clean the fan filter screen on the foot box. Verify standoffs (feet of the foot box) are present. Verify cable tie is present on the power cord. Test current leakage is not more than 500 microamps. Test ground impedance is not more than 0.50 Ohms. Replace cover assembly, top and bottom, every two years. Annual Preventive Maintenance for Joerns Enhanced Bari 10A beds shall consist of the following: Clean and disinfect the exterior of the support surface before start of inspection. Inspect for loose bolts, nuts or pins. Inspect structural parts for any cracking (especially welds), bending or hole enlargement. Inspect tubing or sheet metal for bends or breaks. Ensure casters are secure, swivel properly and are free of debris. Verify caster brakes apply and release properly. Verify steer pedal on both sides of the bed operates properly and steer wheel deploys, operates, and retracts. Ensure all siderails move, latch, and stow properly. Inspect headboard, footboard, and siderail panels for cracks or splits. Ensure all mattress width support extensions move, latch, and stow properly. Ensure mattress length support extension moves, latches, and stows properly. Verify scale functions properly and calibrate as needed. Verify Left and Right emergency manual mattress support platform release functions properly. Verify all bed functions on foot board and hand pendant work properly, including LEDs. Inspect wiring for proper connections and damage. Inspect power cord and plug end for frays or damage. Verify all electrical connections are tight. Verify all ground cables are secure to the bed frame. Test ground impedance is not more than 0.50 Ohms. Test current leakage is not more than 500 microamps. Replace batteries every two years. Inspect pendant control and cord for any physical damage. Inspect actuators for correct mounting at attachment points and ensure all related pins are mounted securely and properly to frame. Lubricate bed frame where required. Inspect bed for rust or corrosion of parts, replace or repaint as needed. Inspect all labels for legibility, proper adherence, and integrity. Clean and sanitize bed frame. Annual Preventive Maintenance for Joerns Arise Therapy Mattresses shall consist of the following: Clean and disinfect the exterior of the support surface before start of inspection. Verify all mattress fasteners are secure. Inspect zipper, snaps, and covers (top, and bottom) are free of tears, cuts, holes, or any other damage. Replace cover if excessive wear is observed. Inspect hose assembly for damage and leaks. Inspect power switch for proper operation. Inspect all electrical connections for tightness and damage. Inspect power cord and plug end for frays or damage. Test ground impedance is not more than 0.50 Ohms. Test current leakage is not more than 500 microamps. Verify control unit buttons function properly. Inspect control unit housing is not cracked or splitting. Remove and clean air filter element on unit housing. Remove mattress cover and ensure air bladders and hoses are not damaged, are secure, and have proper inflation. Inspect all product labeling for signs of degradation. Annual Preventive Maintenance for CHG Sprit Select Platform beds shall consist of the following: Clean and disinfect the exterior of the support surface before start of inspection. Inspect mattress for damage and excessive wear, notify COR if mattress needs replacement. Inspect fiberglass mattress platform for cracked or broken hinges. Perform all bed functions from all control points: footboard staff, patient/staff siderail controls. Verify battery is holding proper charge, replace as needed. Replace batteries every two years. Inspect power cord and plug end for frays or damage. Inspect for pinched, damaged or loose wiring, replace and tighten as needed. Inspect the control boxes and actuators to ensure the enclosures are not cracked or damaged. Inspect the footboard staff control, siderail controls, to ensure the overlays covering the controls are not cracked or damaged. Inspect all grounding wires and equipotential conductor, ensure they are securely fastened to the bed frame. Test ground impedance is not more than 0.50 Ohms. Test current leakage is not more than 500 microamps. Verify nurse call functions properly from all points. Lubricate bed frame where required. Tighten, adjust and/or replace any parts or screws, bolts, clevis/hitch/cotter pins, etc. that are loose or show signs of wear. Ensure all siderails move, latch, and stow properly with no binding and/or grinding. Ensure that contact at any point on each of the six obstruction sensor pads stops all downward bed motion. Ensure bed automatically elevates the 1 safety distance after obstruction is detected. Ensure audible and visual warnings function as designed. Replace any obstruction sensor pad that fails to operate properly. Verify scale functions properly and calibrate as needed. Inspect bed for rust or corrosion of parts, replace or repaint as needed. Inspect all labels for legibility, proper adherence, and integrity. Clean and sanitize bed frame. Schedule All work shall be performed during normal business hours Monday Friday from 7:00 AM 3:30 PM, excluding federal holidays. Unless emergency work is required, no work should be planned on federal holidays. Federal holidays shall be defined as the following dates: New Year s Day, Martin Luther King Day, President s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran s Day, Thanksgiving Day, and Christmas Day. All work activities are not to interfere with patient and/or staff. The Medical Center must maintain its operation 24 hours a day 7 days a week. Therefore, any interruption in service must be scheduled and coordinated with the COR to ensure that no lapses in operation occur. All beds and support surfaces must be inspected on an annual basis to remain in compliance with JACHO. COR will contact the contractor to schedule service for routine repairs as needed. 48-hour call response for emergency repairs is required. Safety Coordinate carefully with COR to avoid disruptions to hospital operation. No tools or equipment shall be left unattended. All work areas must be clean at the close of the day and shall be left in the same condition as before work started.
