Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The associated North American Industrial Classification System (NAICS) code for this procurement is 336211, Motor Vehicle Body Manufacturing, with a small business size standard of 1,000 employees. This is a 100% total Service-Disabled Veteran-Owned Small Business (SDVOSB) set-aside procurement. The Aleda E. Lutz VA Medical Center, 1500 Weiss Street, Saginaw, MI 48602 is seeking to purchase fifty (50) ApneaLink Air Complete Kits. This will include the device, sensor, software, cable, and a 2-year warranty. This combined synopsis/solicitation uses a brand name description of the products required. All interested companies shall provide quotes for the following: Line Item Model Number Description Quantity Unit Price Extended Price 1 22354 1 ApneaLink Air Complete Kit includes Device, Sensor, software, Cable and 2-year Warranty 50 OFFERORS SHALL PROVIDE DESCRIPTIVE LITERATURE, TO INCLUDE CURRENT WARRANTY PROVISIONS TO SUPPORT THE GOVERNMENT S EVALUATION OF THE OFFERED PRODUCT(S). (See Clauses FAR 52.211-6 Brand Name or Equal and VAAR 852.211-72 Technical Industry Standards) This procurement is for new items only; no remanufactured or "gray market" items. No remanufactures or gray market items will be acceptable. Gray market items are Original Equipment Manufacturers (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. Vendor shall be an OEM, authorized dealer, authorized distributor, or authorized reseller for the proposed equipment, verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All warranty and service associated with the equipment shall be in accordance with the OEM terms and conditions. Place of Performance The Contractor shall deliver the quoted items, FOB Destination. Contractor shall indicate on the quote how many days after receipt of an order (ARO) the items can be delivered. Contractor shall deliver the items fully assembled and in operational condition. Delivery shall be to the Aleda E. Lutz VA Medical Center, 1500 Weiss Street, Saginaw, MI 48602. Attn: Respiratory Therapy. Award shall be made to the contractor whose quote offers the best value to the government, considering technical capability, past performance, and price. The government will evaluate information based on the following evaluation criteria: (1) technical capability factor meeting or exceeding the requirement, (2) past performance, and (3) price.
