Description
THIS IS NOT A SOLICITATION FOR OFFERS THIS IS AN AMENDED SOURCES SOUGHT SYNOPSIS TO PREVIOUS POSTINGS. IF A VENDOR HAS PREVIOUSLY RESPONDED WITH THE REQUIREMENTS LISTED BELOW, AN ADDITIONAL RESPONSE IS NOT NECESSARY. This Sources Sought Synopsis is for the purpose of identifying contractors that possess the capabilities outlined below. The NAICS code for this Sources Sought Synopsis is 621111, Offices of Physicians (except Mental Health Specialists), which has an associated small business standard of $11.0 million; the Product Service Code is Q201 Medical Services. The Bureau of Reclamation, Pacific Northwest Region is seeking businesses capable of providing medical monitoring services to ensure that employees are, and remain, physically qualified to perform the essential duties of their position. Employees must perform essential duties without endangering the health and safety of themselves, or others, and to monitor health changes related to work place exposures and activities. Medical surveillance is required when duties of a position are identified, and a determination is made, that the position may have dangerous exposures and/or is arduous as defined in 5 CFR 339.104. The evaluations are used to determine their ability to perform, and continue to perform, the duties of their position. Positions which may be arduous are listed in Reclamation Manual, Directive and Standards, HRM 02-01. The Program includes but is not limited to; a hearing conservation program, respirator medical clearance program, and Fitness for Duty exams and reviews. The contractor shall provide a network of occupational health physicians to conduct testing and examinations for Reclamation employees that require medical monitoring and clearances based on their positions. Due to the sterile, private, and medical importance of the clearances resulting from these tests and exams, contractor staff shall use occupational health/medical personnel and the test and exams shall be conducted at the appropriate medical facility. Testing and examinations shall be reviewed and cleared by an Occupational Health certified physician. The contractor shall provide all required services located throughout the Pacific Northwest Region. The Pacific Northwest Region consists of the states of Idaho, Washington, and Oregon. Additionally, there are small offices located in Alta, Wyoming and Hungry Horse, Montana. Currently, the Pacific Northwest Region has 350 employees in the Program; this number subject to change. The services required include: - Medical surveillance and fitness for duty examinations (including: baseline and periodic) - Regulated Exams (OSHA, DMV, DOT) - Specialty Exams for Crane Operators, Hazardous Waste Workers, Respirator - Clearance, Asbestos, Lead Work, Tower Climbing, and Law Enforcement (Security Response Force SRF) Offices - Accommodation placement examinations and consultation - Physician Consultations - Clinical Laboratory Tests - History/Review of Medical Tests - Medical Imaging/Radiology - Ancillary Diagnostic Services (Audiograms, Cardiac Stress Tests, EKGs, Spirometry, Visual Screening,) - Audiograms and audiometric history comparison to support Hearing Loss Prevention Program (29 CFR1910.95). - Musculoskeletal Evaluation (back, neck, shoulder, hand/wrist, lower extremity assessment) - Immunizations and Vaccinations (Rabies, Tetanus, Hepatitis A and B) - Board Certified Occupational Health Physician /Medical Review Officer Hard copy of records ONLY. Due to the sensitive nature of medical information and Privacy Act requirements, all records will be completed via hard copy ONLY. Appointment information, medical forms, and other pertinent documents will be sent via priority mail or on a secured fax line only. All examinee medical documentation will be sent to the Bureau of Reclamation for maintenance and storage. No examinee medical records will be kept at the service or contractor location. No examinee information or records can be sent electronically or stored on the service providers server or cloud-based data storage. This Sources Sought Synopsis shall not be considered as a request for offer/quote or as an obligation on the part of the Government to acquire any products or services. No entitlement to payment of direct or indirect costs or charges by the Government will arise as a result of the contractor submission of this Sources Sought Synopsis or the Government's use of such information. The Government reserves the right to reject, in whole or in part, any contractor's input resulting from this Sources Sought Synopsis. No contract will be awarded as a result of this posting. Data submitted in response to this posting will not be returned. This notice is for research and information purposes only. Any businesses with the capability to perform the requirement under the applicable NAICS code are encouraged to submit a response to this notice. A courtesy copy of the draft Performance Work Statement is also included with this posting. The Performance Work Statement is in draft form, and for informational purposes only; industry comment is welcome. Interested vendors must submit: 1. Company name, address, DUNS number, and point of contact name and email address; 2. Business size and small business size classification if applicable (e.g. 8(a), HubZone, Veteran Owned, Woman Owned, etc.); 3. Information that would demonstrate the vendor's capability and resources to meet the specifications of the requirement; 4. The vendor's intent to submit a quote if a solicitation is issued. Any company proprietary information must be marked as such, the posting information sent to Reclamation should not exceed a total of 30 one-sided 8 1/2x11 pages, with one inch margins, and font no smaller than 12 point. All responses to this Sources Sought Synopsis posting shall be submitted electronically (via email) to Amanda Somerville at asomerville@usbr.gov no later than Thursday, May 2, 2019.
