Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This solicitation is for services. The solicitation number is 36C247-19-Q-0433, and this solicitation is a Request for Quotes (RFQ). This announcement constitutes the only solicitation, quotes are being requested. The associated North American Industrial Classification System (NAICS) code for this procurement is a 621511 with a small business size standard of $32.5 million. This solicitation is unrestricted open market and only qualified offerors may submit quotes. PERIOD OF PERFORMANCE: One base year with the possibility of four one-year option periods. Base Year: June 1, 2019 May 31, 2020 Option Year 1: June 1, 2020 May 31, 2021 Option Year 2: June 1, 2021 May 31, 2022 Option Year 3: June 1, 2022 May 31, 2023 Option Year 4: June 1, 2023 May 31, 2024 PLACE OF PERFORMANCE: Charlie Norwood VA Medical Center, 1 Freedom Way, Augusta GA 30904 QUOTES SHALL BE EVALUATED: 1) Lowest Price Technically Acceptable 2) Technical Approach 3) Management/Quality Control Approach 4) Relevant Experience 5) Past Performance criteria. Contractor MUST be able to provide: (1) The option to bundle oncology reference laboratory services for Flow Cytometry, Immunohistochemistry and Fluorescence in-situ Hybridization on bone marrow aspirate, peripheral blood, tissue and other fluids. (2) Separate billing for technical and professional services for each case, based on actual usage. Quotes shall be emailed to Donald.Battle@va.gov.
