Description
The Great Lakes Acquisition Center (GLAC) intends to negotiate a sole source contract with BD BIOSCIENCES. for service on the flow cytometer cell sorter (per attached SOW) with the Milwaukee VAMC Building 70, Room Number 70-C-137, Milwaukee, Wisconsin. BD BIOSCIENCES, is the only vendor capable of providing service to their proprietary system. This procurement is being conducted in accordance with FAR 12 & 13 Single- Source Award Justification for Simplified Acquisitions Procedure and no other vendor will satisfy agency requirements. This notice of intent is not a request for competitive quotes. No solicitation documents are available and telephone requests will not be accepted. However, any firm that believes it can meet these requirements may give written notification to the Contracting Specialist by 09:00 AM (CST) April 3, 2019. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to comply with the above requirements. Information must be sent to the Contracting Specialist, Della.Bond@va.gov. TELEPHONE INQUIRIES WILL NOT BE ACCEPTED. Responses received will be evaluated however; a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the Contracting Office will proceed with the sole source negotiation with BD BIOSCIENCES. SCOPE OF WORK (SOW) Background A large group of researchers at the Clement J. Zablocki VAMC use of the flow cytometer and cell sorter on station. This equipment has dramatically increased productivity by decreasing the time and effort currently lost to transporting samples to/from an academic partnering institution with limited system access. This equipment has also further improved experimental reliability, and enabled research studies involving time critical experiments by removing procedural and administrative barriers. Objectives The objective of this procurement is to obtain a service contract to maintain the current cytometer Scope Repair and maintain one (1) cytometer at the Clement J Zablocki VAMC Milwaukee, Research Building 70, Room Number 70-C-137. Tasks or Requirements Coverage includes the BD FACSAria Fusion Special Order System and one BD FACStation (excluding printers) for labor, travel expenses and parts (excluding lasers and consumable items) for the following: - Two (2) Preventive Maintenance Inspections to be performed during a twelve-month period including two (2) PM kits. - Software Revisions released during the Agreement term. - Unlimited service visits, Monday-Friday, excluding BD Biosciences holidays. - Unlimited telephone support for instruments, reagents and applications will be provided at no additional charge. - Every reasonable effort will be made to respond within 48-hours to a request for emergency on-site service Monday through Friday, excluding BD Biosciences holidays.) Selection Criteria The Becton Dickinson Company (BD) is the only source available for this crucial equipment meeting all requirements for repair and service maintenance. Security Access to Room Number 70-C-137 will be by employee escort. Place of Performance Clement J Zablocki VAMC Milwaukee, Research Building 70, Room Number 70-C-137 Period of Performance Continuous as needed. HOURS OF OPERATION Normal hours of operation are Monday through Friday from 8:00 am to 4:30 pm, excluding holidays. Federal and National Holidays observed by the VAMC are: National Holidays Federal Holidays New Year's Day Labor Day Independence Day Memorial Day Thanksgiving Day M L King's Birthday Columbus Day President's Day Veterans Day Christmas Day Also included would be any other day specifically declared by the President of the United States to be a National Holiday. When a holiday falls on a Sunday, the following Monday shall be observed as a legal holiday by U.S. Government agencies. When a holiday falls on a Saturday, the preceding Friday shall be observed as a legal holiday. SECURITY REQUIREMENTS The Contractor s employees shall wear visible identification and company uniforms, approved by the COR at all times while on the premises of the VAMC. It is the responsibility of the Contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police Section or the COR. The VAMC will not invalidate or make reimbursement for any parking violations of the Contractor under any conditions. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to a search. Violations of VA regulations may result in citations answerable in the United States (Federal) District court, not a local district, state, or municipal court. PROTECTION OF GOVERNMENT PROPERTY During work execution, the Contractor shall take special care to protect Government property including furniture, walls, baseboards, and other surfaces. Accidental splashes shall be removed immediately. Damage resulting from the Contractor operations shall be repaired by the Contractor, including painting, refinishing, or replacement, if necessary, at no additional cost to the Government. The Contractor shall be responsible to meet OSHA/Safety requirements in the performance of the work. This shall include, but not limited to, taking all the necessary precautions to protect the patients, visitors, and/or staff at each location. The Contractor shall be held responsible for any injuries and /or damage, which may be caused as a result of the Contractors failure to adhere to these requirements. DAMAGE Extreme caution shall be exercised to prevent damage to the building and its contents. Any damage must be reported to the COR immediately for appropriate action. Any damage caused by the contractor will be repaired and/or replaced to the satisfaction of the VA at the Contractor s expense. FACILITY CHECK-IN REQUIREMENTS The Contractor shall be required to report to the Facilities Management Office, Bldg 70, Wing E or the Boiler plant, Bldg 112 to log in for services performed under this contract. This check in is mandatory. The FSE shall also be required to log out with the above or as specified by the COR. ALL reports shall be submitted to the COR for an Acceptance Signature. If the COR is unavailable, a signed, authorized copy of the report will be sent to the Contractor after the work can be reviewed, if requested or noted on the report. DEFINITIONS/ACRONYMS Acceptance Signature VA employee who is authorized to sign-off on the ESR, which indicates that the PM has been concluded or is still pending completion, or that the Emergency Repair has been accomplished or is still in a pending status. Authorization Signature the COR s signature; indicates that the COR accepts work status as stated on the ESR. CO Contracting Officer COR Contracting Officer s Representative ESR Employee/Vendor Service Report A documentation of the services rendered for each incidence of work performance under the terms and conditions of the contract. FSE Field Service Employee - A person who is authorized by the Contractor to perform services on the VAMC premises. VAMC Department of Veterans Affairs Medical Center GENERAL INFORMATION Smoking Policy Effective January 1, 2019, the Clement J Zablocki VA Medical Center is a smoke free and tobacco free campus. Employees and contractors are not allowed to smoke anywhere on the campus A copy of hospital smoking policy is available upon request. Violation of the VAMC Milwaukee smoking policy may result in worker being removed from worksite for duration of project.
