- This is a Sources Sought notice and not a request for quotes. This announcement is to seek qualified Service Disabled Veteran Owned Small Business firms and/or Veteran Owned Small Business Firms interested is submitting potential Patient Aligned Care Team (PACT) Primary Care Physician candidates under the North American Classification System (NAICS) Code of 561320, Temporary Help Services. Responses to this Sources Sought will be used by the Government to make appropriate acquisition planning decisions. VA Central Western Massachusetts Healthcare System (CWMHCS) has a requirement to obtain two (2) Full Time Equivalent Employees (FTEE) to provide Patient Aligned Care Team (PACT) Primary Care Physician services. The place of performance shall be one of two Community Based Outpatient Clinics (CBOCS) located at: Pittsfield CBOC -73 Eagle St, Pittsfield, MA 01201 or Greenfield CBOC - 143 Munson St., Greenfield, MA 01301. The primary tour of duty will be Monday through Friday from 8:00am-4:30pm, excluding federal holidays Note: Contractor will be paid for actual hours worked only. Potential Sources who are interested and capable of providing the above services must demonstrate their experience and knowledge of the required services and the capability to provide such services. Potential sources should identify business name, address, DUNS number, point of contact, business size and business socio-economic category SDVOSB, VOSB, registered and active in VIP (http://www.vetbiz.gov), service capability statement, information for recent and relevant acquisitions similar in nature, GSA Contract Number (if applicable), and any additional information felt to be pertinent. The NAICS code for this procurement is 561320 and the small business size standard is $27.5. Written responses to this notice shall be electronically mailed in PDF format or word format to jaime.hudgins2@va.gov. Telephone responses will not be accepted. Written responses must be received no later than 1:00 PM (EST), Monday, 18 February 2019. This Sources Sought is to facilitate the Contracting Officer s review of the market base, for acquisition planning, size determination, and procurement strategy.
