- Draft Statement of Work Send a capability statement, no more than one page in length; to include Company Name, Duns Number, Socioeconomic Category, brief description of your business entities capability to handle this particular project in the Pittsburgh area, website and GSA Contract number & or other VA Medical Center contract information. BACKGROUND Annual maintenance and repairs of Siemens imaging equipment - Magnetom Espree 1.5T MRI and the Magnetom Verio 3.0T MRI. It also includes the MR Chiller Model KCC/KSC 215 at function location 400-214235, MR Chiller Model KCC/KSC 215 at function location 400-2296931 and the MR Chiller Model KCC/KSC 215-2K at function location 400-296931. The contractor shall provide repair services, which may consist of calibration, cleaning, oiling, adjusting, replacing parts, etc., and maintaining the equipment including all intervening calls necessary between regular services and calibrations. All required parts should be furnished. Contractor shall ensure that the equipment functions in conformance with the latest published edition of NFPA-99, OSHA, and the Original Equipment Manufacturer s (OEM) performance standards and specifications. Preventative maintenance (PM) shall be carried out according to the manufacturer's recommended schedule. Planned maintenance includes checking mechanical and electrical safety, lubrication, functional testing and adjusting for optimum performance as specified in the detailed planned maintenance work plan. Contractor will supply at its own expense, necessary parts, except as indicated in the Glossary section, provided replacement of the parts is required because of normal wear and tear or otherwise deemed necessary by the contractor and further provided that the Siemens-manufactured parts are available from the factory. All parts will be new, standard parts, or used, reworked or refurbished parts that comply with applicable performance and reliability specifications. Exchange parts removed from the equipment shall become the property of the contractor unless such exchange parts constitute hazardous wastes, hazardous substances, special wastes or other similar materials, as such terms are defined by any federal, state or local laws, rules or regulations, in which case, at the option of Siemens, the exchange parts shall remain the property of the Customer and shall be disposed of by the Customer in strict compliance with all applicable laws, rules and regulations. System updates whenever the equipment covered by this Agreement utilizes OEM operating system software, contractor will provide all maintenance and commercially available updates for such operating system software as part of this Agreement. Such updates will solely enhance previously purchased capacities of the equipment. Operating system software upgrades that provide new features or capabilities or that require hardware changes will be offered to Customer when commercially available and at purchase prices established by OEM. In addition, some upgrades may require applications training performed by OEM personnel that will be offered at OEM rates and terms then in effect. Nothing in this Agreement shall in any way grant to Customer any right to or license in any diagnostic service software utilized by the contractor in servicing the equipment. Such service software is and remains the property of the contractor and is available to Customer pursuant to the terms and conditions of a separate diagnostic materials license agreement, which may require payment of a license fee. This service software shall be disabled by the contractor upon cancellation or termination of this Agreement. Access to technical telephone support and remote diagnostics for connected equipment. Contract Excludes: Parts defective due to acts of God, abuse, neglect, thermal and shock. Specialty components, including, but not limited to: glassware, flat detectors, consumables, transducers, MRI coils, SPECT and PET sources (unless purchased as an option). Non-Siemens components and accessories such as VCR, injectors, laser printers, MR surface coils, tables/table tops, UPS, etc.) unless specified below. Service Calls: The CO, COR or designated alternate has the authority to approve/request a service call from the contractor. CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting, or has advised the contractor that a kick off meeting is waived. GENERAL REQUIREMENTS Contractor shall notify Biomedical Engineering and the Police service when they arrive on site. Contractor will schedule with Biomedical Engineering all preventative maintenance and repairs. Contractor will respond to repair requests either in person (within 4 hours) or via phone call (within 30 minutes) during normal business hours. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES General Task Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described below. Task One: Maintenance will be scheduled with VAPHS staff before coming on site and will be performed by staff trained on these devices. Deliverable One: A record, either hard copy or electronic copy, detailing the work performed in task one. Task two: The contractor shall provide parts and labor related to any repairs for the unit should it malfunction. A phone number to call for tech support will be made available for notification of the need of repair. Deliverable Two: A record, either hard copy or electronic copy, detailing the work performed in task two. Draft copies of deliverables shall be provided to the Biomedical Engineering staff at VAPHS before the contractor leaves the facility at each visit. If a final copy can be provided at that time then a draft copy is not necessary, final copies must be provided within 5 business days of each visit. CONTRACTOR EXPERIENCE REQUIREMENTS All personnel providing service to VAPHS equipment must have original equipment manufacturer (OEM) training certification due to the complexity of our machines. INFLUENZA PREVENTION VHA Directive 1192 requires all health care personnel (HCP) to participate in the seasonal influenza prevention program and outlines the key implementation steps. Health care personnel is defined in VHA Directive 1192 as all VA licensed and unlicensed, clinical and administrative, paid and unpaid, full- and part-time employees, intermittent employees, fee basis employees, VA contractors, students, researchers, volunteers, and trainees who work in VHA locations. HCP covered by the policy are expected to receive annual influenza vaccination. This requirement is extended to all Contractor personnel with potential to come into minimal contact (passing in the corridor) with any patients, visitors, or staff members at VAPHS. Those individuals unable or unwilling to be vaccinated are required to wear a face mask throughout the influenza season. Masks are available at entrances throughout the medical center. It is the responsibility of the contractor to ensure that all contract staff is compliant with the requirements outlined in VHA Directive 1192. The contractor shall maintain the following documentation: - Documentation of vaccination, e.g., signed record of immunization from a health care provider or pharmacy, or a copy of medical records documenting the vaccination. - Completed Health Care Personnel Influenza Vaccination Form (Attachment B of VHA Directive 1192) if unwilling or unable to receive the vaccine. The contractor is required to submit annual certification in the form of a memorandum to the Contracting Officer s Representative (COR) that all contract staff performing services at VA facilities is in compliance with VHA Directive 1192. TUBERCULOSIS PREVENTION Standard Personnel Testing (PPD, etc.): Contractor shall provide proof of the following tests for personnel within five (5) calendar days after contract award and prior to the first duty shift to the COR and Contracting Officer. Tests shall be current within the past year. TUBERCULOSIS TESTING: Contractor shall provide proof of a negative reaction to PPD testing for all contract personnel. A negative chest radiographic report for active tuberculosis shall be provided in cases of positive PPD results. The PPD test shall be repeated annually. RUBELLA TESTING: Contractor shall provide proof of immunization for all contract personnel for measles, mumps, rubella or a rubella titer of 1.8 or greater. If the titer is less than 1.8, a rubella immunization shall be administered with follow-up documentation to the COR. Please provide a signed memo stating that: (number of assigned Key Personnel) assigned to VA Contract #___________ have tested negative for tuberculosis within the past 365 days. (number of assigned Key Personnel) assigned to VA Contract #___________ have been immunized for measles, mumps, rubella or a rubella titer of 1.8 or greater.
