Description

#2 Diesel Fuel delivered to the Rosebud IHS Hospital, Rosebud, SD. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Rosebud Indian Health Service (IHS) intends to award a Firm Fixed-Price, Non-Personal Service Delivery Order in response to Request for Quote (RFQ) 19-RB-008. This is Small Business Set Aside. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-01. The associated NAICS code is 454310 and small business size standard is 100 employees. The Contract Line Item (CLIN) and pricing structure are as shown below. CLIN 0001 (Date of Award - May 31, 2019) - #2 Diesel Fuel 20,000 Gallons @ $________ per Gallon = $__________ The Rosebud IHS Hospital is requesting two (2) deliveries of up to 10,000 gallons each. The first delivery at the time of the award and the second before May 31, 2019. The delivery and acceptance term for this order is FOB Destination. Complete attached Standard Form 18. Attached are the Federal Acquisition Regulations (FAR) and the Health & Human Services Acquisition Regulation (HHSAR) clauses that are applicable including the following: FAR 52.212-1, Instructions to Offerors - Commercial (Oct 2018), Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted on the SF 18, letterhead stationery, or as otherwise specified in the solicitation. As a minimum, offers must show -- (1) The solicitation number RFQ-19-RB-008; (2) Closing date of February 5, 2019 at 11:00 AM CST; (3) The name, address, and telephone number of the offeror and email address of the contact person; (4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include documents, if necessary; (5) Terms of any express warranty; (6) Price schedule, as stated on SF-18; (7) "Remit to" address, if different than mailing address; (8) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically); (9) Acknowledgment of Solicitation Amendments; (10) Past performance information, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information); and (11) Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.
