Page 1 of 3 This notice is for Solicitation Number 36C24219B0016 - 561A4-12-150 Repair Brick Façade B-135 at the New Jersey Healthcare System Lyons Campus, 151 Knollcroft Road, Bldg. 135, Lyons, NJ 07939 This shall be a Service Disabled Veteran Owned Small Business (SDVOSB) Set-Aside. As a result, all SDVOSB's may submit a Proposal, which will be considered by the agency. This project is classified under NAICS Code 236220, and has a Size Standard of $36.5 million. The cost estimate range for this project is $500,000 and $1,000,000. STATEMENT OF WORK Repair Brick Façade B-135 PROJECT NO. 561A4-12-150, VANJHCS (Lyons Campus) 1.Contract Title. Furnish construction services to include management, labor, equipment and all material to meet the design and specifications for the Repair Brick Façade B-135 project at the VA NJHCS, Lyons Campus. 2. Background. The existing brick facade is separating in various locations on the building. The brick parapet is loose and is deteriorating in several locations. In some areas the deterioration is allowing water to infiltrate the interior of the building. This condition presents a hazard to anyone walking close to the perimeter of the building. Safety fence has been installed to keep pedestrian traffic at a safe distance from the perimeter where brick could become dislodged and fall. 3. Scope: Furnish construction services to include management, labor, equipment and all material to meet the design and specifications to replace all listed as follows: Task 1: Replace cast stone joints at building exterior - Remove existing sealant / caulk, backer rod and grout around all exterior capstone joints - Clean dirt and debris from inside joints - Replace sealant / caulk, backer rod and grout around all exterior capstone joints Task 2: Replace joints at exterior windows - Remove existing sealant / caulk around all exterior windows and cast stone sills - Clean dirt and debris from inside joints - Replace sealant / caulk around all exterior windows and cast stone sills - Clean cast stone sills - Clean windows from any dirt or debris from construction Task 3: Repair brick at building exterior - Remove brick courses at parapet (courses from capstone to parapet flashing, APPROX. 12 courses) - Clean out any grout and mortar. - Install proper flashing - Install brick with weep holes every 2'-0" O.C. - Ref: 4/AE500 Task 4: Repair portion of entry canopy roof - Remove existing roof membrane and insulation - Install new tapered insulation to existing roof drains. - Install new roof membrane - Ref: AE500 for details - NOTE: Entrance must remain open at all times. Reference Civil drawings for scaffolding layout. Task 5: Replace joints at clerestory windows - Remove existing sealant / caulk around exterior clerestory windows - Clean dirt and debris from inside joints - Replace sealant / caulk around all exterior windows - Clean windows from any dirt or debris from construction Task 6: Correct leaning parapet wall - Install #4 bar in core of 8" and 4" CMU blocks - See ROOF PLAN for location - See sheet S100 for details Task 7: Clean capstone at building exterior - Remove lime run and efflorescence from face of brick - Replace sealant / caulk, backer rod and grout at joints as required Task 8: Clean brick façade - Remove lime run and efflorescence from face of brick - Repair or replace cracked and damaged brick - Tuck point areas with mortar failure 4. Specific Tasks: a. Demolition: Provide all labor, equipment and material as outlined in the construction package to include construction fencing and material disposal. Disposal/Recycle manifest shall be provide to the COR monthly b. Construction: Provide all labor, equipment and material as outlined in the construction package to include to complete the project in a timely and professional manner to include phasing to allow access to buildings at all times. All work to be completed in accordance with all local, state and federal regulations. 5. Performance Monitoring (if applicable). The designated COR will monitor the contract performance and make sure the contractor adheres to VA standards and designated project milestones as well as provide all necessary support required to complete the subject design. 6. Security Requirements. The contractor employees will be required to apply for PIV flash badges. 7. Inspection and Acceptance Criteria. The designated COR will certify the work done under the contract shall be performed to time and standard. Designated COR is also responsible to assure the inspection and acceptance of products provided incidental to services. 9. Risk Control The chosen contractor shall provide a phasing and ICRA (infectious control risk assessment) drawing and specifications section. Drawing and specs shall determine vulnerable areas and their pertaining level of risk and methods of infections controls measures. The contractor will provide a site specific safety plan. The contractor will provide all credentials for workers involved with the ACM abatement. The contractor will provide all OSHA credentials for workers as outlined in the specifications. 10. Place of Performance. Place of performance shall be at 151 Knollcroft Road, Lyons, NJ 07939 11. Period of Performance. 6 Months from the NTP date.
