- This is a combined synopsis/solicitation for commercial items or services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is 19-242-SOL-00024 and is issued as a Request for Quotes (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The associated North American Industrial Classification System (NAICS) code for this procurement is 511210 with a small business size standard of $38.50. This requirement is a 100% Set-Aside for Indian Small Business Economic Enterprise (ISBEE) concerns with a cascading set-aside for Other Small Business Concerns and only quotes from qualified offerors will be considered. See Clause titled, "Cascading Set-Aside" for more information. Only one award will result from this solicitation. SCOPE OF WORK PURPOSE / PLACE OF PERFORMANCE The purpose is to provide Policy and Procedure storage and maintenance. This Electronic Policy Management System is being requested for the Albuquerque Indian Health Service and its multiple clinics and service units. Services are to be performed through the Albuquerque Area Office, which is located at 4101 Indian School Road NE, Suite 225, Albuquerque, NM 87110. The Albuquerque Area Office enters and maintains working policies for the Albuquerque Area Office, ACL Hospital, Albuquerque Service Unit, Jicarilla Service Unit, Mescalero Service Unit, New Sunrise Regional Center, Santa Fe Service Unit, Ute Mountain Ute Health Center, Taos-Picuris Health Center and Zuni Service Unit. REQUIREMENTS The contractor shall furnish all parts, supplies, material, labor, equipment and supervision necessary to provide an Electronic Policy Management System. The contractor will be required to provide a one-year base contract with four one-year options for the delivery of an electronic policy management system from an independent contractor. PERIOD OF PERFORMANCE Base Year: 02/01/2019 - 01/31/2020 1st Option Year: 02/01/2020 - 01/31/2021 2nd Option Year: 02/01/2021 - 01/31/2022 3rd Option Year: 02/01/2022 - 01/31/2023 4th Option Year: 02/01/2023 - 01/31/2024 ***Please submit proposals to include base & option year totals as well as total contract price*** SCHEDULE OF SERVICES Hours of business support are Monday through Friday 8AM to 4:30 PM, excluding official government holidays. The contractor shall provide the following: 1. Policy management system graphical user interface shall be compliant with Section 508 of the Rehabilitation Act published by the U.S. Access Board, 36 Code of Federal Regulations 1194, available at www.access-board.gov/sec508/standards.htm. 2. All data conversions, records created (i.e. assembled databases) shall be the sole property of the Albuquerque Area Indian Health Service. 3. Backup copies of all policies shall be continuously available through electronic formats. 4. System architecture must be for regional (Area Office) and local (Service Units) site administration with capability for expansion. 5. System shall have dashboard page showing all delinquent and upcoming policy revisions. 6. System shall provide automated communications of status of pending and approved policies, via e-mail. 7. System shall retain all electronic copies of past policy versions. 8. System shall have ability to implement, track, and report online customer-configurable questionnaires to verify user knowledge of policies. 9. Policy management system software upgrades shall be included in the service agreement. REFERENCE The contractor is not able to access the IHS health information of patients. Patient health information is confidential in accordance with the Privacy Act of 1974. QUALITY CONTROL STANDARDS The Government reserves the right to inspect and evaluate all services provided during the performance of this contract and may conduct reviews or evaluations at any time during the performance of this contract. Evaluations are documented by the Program Manager or COR. If an evaluation indicates defective performance, the COR notifies the Contracting Officer (CO). The CO may issue a Discrepancy Report to the contractor. The contractor shall address any Discrepancy Report in writing to the Contracting Officer within ten calendar days of notification, indicating any corrective action taken to rectify the deficiency. SCHEDULE OF DELIVERABLES Delivery and accountability of work may be documented upon request by the COR as needed and / or required. Contractor shall provide all materials and labor for maintaining the electronic policy management system.
