- B.2 STATEMENT OF WORK GENERAL INFORMATION 1. Title of Project: Air Defense Super Odor Control 2. Scope of Work: Battery-operated, wall-mounted odor control units installed in patient care areas restrooms, eliminates and provides 24-hour odor control protection. These units dispense fragrance continually. Contract will allow VAMC Northport to rent 100 units, while contractor is responsible for all maintenance, including replacing batteries, refills and broken units. The odor neutralizer and dispensing system must be programmable with 24-hour time-release protection. All units must be serviced regularly on a monthly basis by a technician. It is at the discretion of the contractor whether to service all units during one monthly visit or to make numerous visits to cover all areas; however, all units must be checked once per month. Each unit must cover at least 6,000 cubic feet of air. A log of all units serviced during each site visit must be submitted to COR. All fragrances are acceptable unless notified otherwise by the Contracting Officer or COR. However, as the setting for this service is in a hospital environment, the contractor should take special care to select a fragrance that is not overpowering or a potential irritant to patients or staff. 3. Background: This service has been performed at our facility for many years. This contract is a bona fide need for the benefit of patient care. Current wall units are mounted and cannot be replaced without extensive damage and costly repair. VAMC Northport is willing to accept different wall units then the current model, but the contractor must patch any damage caused by in stallion of any new units. 4. Performance Period: Base year service is expected to begin on 12/17/2018 to 12/16/2019. Afterwards, requesting option years covering 12-month periods for 4 years: Base Year: 12/17/2018 to 12/16/2019 Option year 1: 12/17/2019 to 12/16/2020 Option year 2: 12/17/2020 to 12/16/2021 Option Year 3: 12/17/2021 to 12/16/2022 Option Year 4: 12/17/2022 to 12/16/2023 5. Type of Contract: This is a new firm-fixed-price contract. Response date (deadline): VAMC Northport needs this contract in place as soon as possible to prevent lapse in service that adversely affects patient care and well-being.
