- This is a combined synopsis/solicitation for commercial supplies/services prepared in accordance with the format in Subpart 12.6, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a request for quotes under solicitation number: 36C26219Q0316 The Department of Veterans Affairs Contracting Office has a requirement for Hemodialysis Supplies that meets the following salient characteristics with brand name and manufacturer. The Contractor shall furnish all supplies/services at The VA West Los Angeles Healthcare System. SL-2010M2096 1 ITEM ID NO. 48477 BLOODLINES-STREAMLINE-32/CS-DIALYSIS PKG: 1 per EA 640 CS 803 2 ITEM ID NO. 43433 BICARBONATE, DIALYSIS, POWDER,720 GRAM, CARTRIDGE, BICART 10 PKG: 1 per EA 1900 CS 8001 3 ITEM ID NO. 24940 SOLUTION, HEMODIALYSIS,50% CITRIC ACID, LIQUID,10 LITER, STERILE PKG: 1 per EA 250 EA 7107367 4 ITEM ID NO. 39195 FILTER ASSEMBLY, DIALYSIS UNIT, POLYSULFONE MEMBRANE, FOR THE DIALOG ULTRAPURE FLUID SYSTEM PKG: 1 per EA 40 CS FUNCTIONAL/PERFORMANCE WORK STATEMENT 1. Performance Period: January 25, 2019 January 24, 2020 2. Contract Title: Dialysis Medical Supplies 3. Background This purchase request is for critical dialysis medical supplies to provide dialysis therapies on veterans with both chronic and acute kidney failures for B-500 (outpatient, inpatient, acute TX in ICU) and B-213 (outpatient) at VA Greater Los Angeles Healthcare System. Each dialysis patient must be dialyzed at least for 4 hours per TX, 3 times a week to sustain their lives, and therefore, it is extremely important to have all these supplies on hand when needed. 4. Specific Tasks The Contractor will provide all items listed below specifically manufactured to fit and operates only on Dialog Plus machines. Delivery The vendor will deliver supplies to VA GLAHS, building 500, room 6013 on an as needed basis when the Dialysis Unit contacts the vendor with an order. The solicitation will be in accordance with FAR Part 12, 13, and 19, Acquisition of Commercial Items, Simplified Acquisition Procedures, Small Business Programs, and Veteran Affairs Acquisition Regulations as supplemented with additional information in this notice. The North American Industrial Classification System (NAICS) code for this requirement is 334510 Size Standard 1000 Employees. The Contractor shall adhere to all federal and state laws and regulations in effect during the term of this contract. The Government intends to award a firm-fixed-price contract. The Contract begins on the date of award. Responses to the solicitation shall be due on Thursday, January 24, 2019 at 12:00 PM PST.
