Description
This is a combined synopsis/solicitation for Commercial Items in accordance with the format prescribed in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are requested and a written solicitation will not be issued. The solicitation is being issued as a Request for Quote (RFQ); solicitation number is 36C262-19-Q-0274, and a firm fixed price contract is contemplated. This procurement is SDVOSB SET ASIDE with the BRAND NAME OR EQUAL for the Med Maps system and the associated North American Industry Classification System (NAICS) code is 541430 Graphic Design Services with a business size standard of $7,500,000.00. Offers from capable and responsible sources that are authorized by the OEM will be considered. Firms offering a response to this notice should ensure that their VIP verification is active and their registration in the System for Award Management (SAM) database (https://www.sam.gov/portal/public/SAM/) is current and should ensure that the applicable NAICS code (North American Industry Classification System (NAICS) code is 541430 Graphic Design Services with a business size standard of $7,500,000.00) is included in the SAM profile prior to submission of quotations. NOTE: Interested parties are to submit their Capabilities Statement to the Contracting Office (No more than 5 pages, please) via email to omar.majette@va.gov no later than 01/22/2019 at 1:00pm PST. The following Services are required: ITEM UNIT PROPOSED PRICE TOTAL PRICE CLIN 0001 40 Hours of Edits for Campus Maps EA CLIN 0002 (14) Wall Maps - 22" x 34" - (2) Elevator Lobby (Map & Directory), (2) SCI Atrium Clinics (elevator), (2) 2nd Floor Elevator, (2) 3rd Floor Elevator, (2) 4th Floor Elevator, (2) 5th Floor (2) Elevator and Directory EA CLIN 0003 4) Wall Maps - 45.5" x 45.5" - (2) Front Lobby and (2) SCI Atrium EA CLIN 0004 Maintenance Agreement to include hosting for the interactive map as well as 9 hours of edits EA STATEMENT OF WORK TITLE: CAMPUS MAP DIRECTORY SERVICES. BACKGROUND INFORMATION: This is for immediate services rendered for making additions to the map, correcting the directory and printing appropriate changes, along with yearly maintenance. OBJECTIVES: The purpose of this contract is to have contractor update our facility mapping systems online and throughout the San Diego VA Medical Center. PERIOD OF PERFORMANCE: The period of performance is anticipated to be from February 01, 2019 through January 31, 2020. PLACE OF PERFORMANCE: VA San Diego Healthcare System 3350 La Jolla Village Dr. San Diego, CA 92161 SPECIFIC REQUIREMENTS: a. The contractor shall provide 40 Hours of Edits for San Diego VA Campus Maps b. Destination Nov 1991 52.211-6 -- Brand Name or Equal As prescribed in 11.107(a), insert the following provision: Brand Name or Equal (Aug 1999) (a) If an item in this solicitation is identified as brand name or equal, the purchase description reflects the characteristics and level of quality that will satisfy the Government s needs. The salient physical, functional, or performance characteristics that equal products must meet are specified in the solicitation. (b) To be considered for award, offers of equal products, including equal products of the brand name manufacturer, must (1) Meet the salient physical, functional, or performance characteristic specified in this solicitation; (2) Clearly identify the item by (i) Brand name, if any; Med Maps Proprietary Information for Interactive Maps. (ii) Make or model number; (3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and (4) Clearly describe any modification the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modification. (c) The Contracting Officer will evaluate equal products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. (d) Unless the offeror clearly indicates in its offer that the product being offered is an equal product, the offeror shall provide the brand name product referenced in the solicitation. FAR 52.217-7 Option for Increased Quantity - Separately Price Line Item Mar 1989 The Government may require the delivery of the numbered item, identified in the Schedule as an option item, in the quantity and at the price stated in the Schedule. The Contracting Officer may exercise the option by written notice to the Contractor within 10 days before contract expires. Delivery of added items shall continue at the same rate that like items are called for under the contract, unless the parties otherwise agree. 52.252-2 Clauses Incorporated by Reference Feb 1998 This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. Also, the full text of a clause may be accessed electronically at this/these address(es): https://www.acquisition.gov/browsefar The following VAAR clauses are also applicable to this acquisition: VAAR 852.203-70 COMMERCIAL ADVERTISING (JAN 2008) The bidder or offeror agrees that if a contract is awarded to him/her, as a result of this solicitation, he/she will not advertise the award of the contract in his/her commercial advertising in such a manner as to state or imply that the Department of Veterans Affairs endorses a product, project or commercial line of endeavor. (End of Clause) VAAR 852.237-70 CONTRACTOR RESPONSIBILITIES (APR 1984) The contractor shall obtain all necessary licenses and/or permits required to perform this work. He/she shall take all reasonable precautions necessary to protect persons and property from injury or damage during the performance of this contract. He/she shall be responsible for any injury to himself/herself, his/her employees, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by his/her employees fault or negligence, and shall maintain personal liability and property damage insurance having coverage for a limit as required by the laws of the State of California. Further, it is agreed that any negligence of the Government, its officers, agents, servants and employees, shall not be the responsibility of the contractor hereunder with the regard to any claims, loss, damage, injury, and liability resulting there from. (End of Clause) VAAR 852.246-70 GUARANTEE (JAN 2008) The contractor guarantees the equipment against defective material, workmanship and performance for a period of, said guarantee to run from date of acceptance of the equipment by the Government. The contractor agrees to furnish, without cost to the Government, replacement of all parts and material that are found to be defective during the guarantee period. Replacement of material and parts will be furnished to the Government at the point of installation, if installation is within the continental United States or f.o.b. the continental U.S. port to be designated by the contracting officer if installation is outside of the continental United States. Cost of installation of replacement material and parts shall be borne by the contractor.
