Description
Good afternoon, Synopsis: Please note that this is NOT a request for quotes or proposals, the Government is seeking information for market research purposes only. Network Contracting Office (NCO) 2 of the Department of Veterans Affairs is currently conducting a market survey for qualified firms capable of Service for LTV 1200 Ventilators PM The North American Industry Classification Systems (NAICS) is 811219.   Responses to this notice must be submitted in writing (email or fax) and must be received no later than August 18, 2018 at 12:00 Noon. Email: NATALIE.ACEVEDO@VA.GOV and Fax: 718-741-4722. No telephone inquiries will be accepted. Interested parties MUST provide company/individual name, a capability statement, examples of same or similar work performed at other facilities, DUNS number and address, point of contact and social-economic. Interested parties who do not submit the above information on or before the response due date and time will not be considered for this procurement.   Please review the attached Statement of Work attached herein. **Qualified Technicians Only service technicians who have been trained and certified by CareFusion to service the LTV 1200 ventilator are qualified to perform the 10,000 and 30,000-hour maintenance on the LTV 1200. ** Provide documentation of training and certification The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, women owned small business, HUB Zone, 8(a), small business or large business), relative to NAICS 811219 Responses to this notice will be used by the Government to make appropriate acquisition decisions. A solicitation is not currently available. If a solicitation is issued, it will be announced on GSA E-buy website http://www.fbo.gov   at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. STATEMENT OF WORK GENERAL INFORMATION 1. Title of Project: One Time 30k hour Preventive maintenance of Carefusion LTV 1200 (total of 6 units) 2. Scope of Work: The contractor shall provide all resources (labor, equipment, tools, materials, preventive maintenance service, and parts) necessary to accomplish a 30k hour preventive maintenance as described in this statement of work (SOW). This will be a one-time preventive maintenance for 6 Carefusion LTV 1200 units. Equipment: Manufacturer: Carefusion Device Name: Ventilator ID#: 57182, 57183, 57184, 57185, 57186, 57187 Model: LTV 1200 Serial# E06457, E06931, E07055, E07096, E07142, E07155 (respectively) Site ID# N/A VA Contract #: N/A Work Order#: Comments: Estimated cost. Travel, shipping, labor and parts are included in estimate. Any additional charges (parts or services) claimed must be approved by the Contracting Officer Technical Representative (COR) before service is completed. 3. Background: Explanation of issue Using Service: Respiration Therapy Description of service: One-time preventive maintenance is required. Bronx VA was donated these units; however, they cannot be utilized until a proper PM has been performed. Is Patient Care Affected: Yes, these units are portable ventilators that are needed but cannot be distributed until a preventive maintenance has occurred. 4. Performance Period: Normal business hours: [Monday - Friday] from [7:00am to 6:00pm], excluding federal holidays. Federal holidays are: New Year s Day Labor Day Martin Luther King Day Columbus Day President's Day Veterans Day Memorial Day Christmas Day Thanksgiving Day Independence Day And any other day specifically declared by the President of the United States to be a national holiday. Type of Contract: Contract Type: Non-Repetitive Firm Fixed Price 6. Additional Requirements: Preventive maintenance procedure must follow the procedures and requirements set by the Original Equipment Manufacturer (OEM). The procedure performed on the equipment must be included in the documentation of services performed. This includes: Recharge the Internal Battery by plugging the ventilator into an AC power source for 24 hours. Check the Inlet Filter, clean if necessary. Check the Fan Filter, clean if necessary. Check the ventilator per vendor specified Ventilator Checkout Tests. While the Perform the Power (external) disconnect test Verify Vte or VE monitor Verify airway pressure or PIP monitor per manufacturer standards Verify delivered O2 concentration if not using an oxygen analyzer continuously, Replace the Internal Battery only with CareFusion battery P/N 18608-001. Calibrate the Transducers. Replace the Power Board. Replace the Sounder Assembly. Clean or replace the Interior Air Inlet Filter. Clean or replace the O2 Inlet Filter. Replace the Turbine Manifold Assembly. Replace the Solenoid Manifold. Replace the Flow Valve. Replace the Rotary Switch Assembly. Replace the O2 Blender. Replace the Fan Assembly. Replace all Silicone Tubing. Check the Thermo Pads for compression and replace if necessary. Full calibration of the unit Available software updates and upgrades Preventive maintenance must be performed by a Pulmonetic Systems factory trained service technician All service/repairs will be performed during normal business hours of coverage unless requested and approved by [COR]. The contractor shall complete the work required under this SOW in 30 calendar days or less from date of award, unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor s proposal, the contractor s proposed completion date shall prevail. Documentation: The contractor must provide documentation of services provided as described below: The documentation will include legible detailed descriptions of the services performed, including replaced parts and costs. Such documentation shall meet the guidelines as set forth in the Conformance Standards Section. All records and logs shall be turned over to the COR or Biomedical Engineering Designee at the completion of the work performed. The documentation at a minimum, shall document the following data legibly and in complete detail: A. Name of Contractor. B. Name of Field Service Engineer (FSE) who performed services. C. Contractor Service Number/Log Number. D. Date, Time (starting and ending), Equipment Downtime and Hours-On-Site for service call. E. Description of Problem Reported by COR/User. F. Identification of Equipment to be serviced: INV. ID#, Manufacturer s Name, Device, Model#, Serial#, and any other Manufacturer's ID# s. G. Itemized Description of Service Performed (including Costs associated with after normal working hour services, if applicable), including: Labor and Travel, Parts (with parts) and Materials and Circuit Location of problem/corrective action. H. Total Cost to be billed. I. Signatures: 1. FSE performing services described. 2. Biomedical Engineering Employee who witnessed service described. J. Equipment downtime All parts supplied shall be compatible with existing equipment. The contractor shall use OEM new parts only. New parts shall be furnished by the contractor and guaranteed against defects and/or failure for a period of 1 year from the date of completed installation of the component. Used parts, shall not be installed without approval by the COR. The contractor shall provide to the contracting officer and his/her designee with written proof that all gauging and measuring equipment used to check and calibrate the equipment has been calibrated according to the specifications of all applicable standards.
