- This is a sources sought notice; this is neither a solicitation nor a promise of future work. The Wilmington VAMC is seeking SDVOSB/VOSB and any Small or large company regardless of Socio-economic category that can provide the requirement as spelled out in the Statement of Work (SOW) below. The veteran business must be registered in SAM and certified in VETBIZ (VIP). The small or large businesses must be registered in SAM. All Vendors interested in, and with the capability and capacity to fulfill this requirement, are required to furnish the following minimum information in reply to the sources sought: Company Name, address, Government Business Point of Contact, telephone number and email address. Company DUNS number, CAGE Code, and Tax ID numbers. Identify any current Federal Supply Schedule, GSA Contracts held, if none then state that no FSS/GSA schedules are held. Responses shall be submitted electronically, via email to Donna.Toliver, at donna.toliver3@va.gov. Responses are due no later Thursday 01/04/2019 at 1:00 pm. STATEMENT OF WORK PRODUCE WILMINGTON VA MEDICAL CENTER STATEMENT OF WORK PART A GENERAL INFORMATION A.1 INTRODUCTION This is to establish a Blanket Purchase Agreement for fresh and seasonal produce for patient/resident meal trays at the Wilmington VA Medical Center located at 1601 Kirkwood Highway, Wilmington, DE. A.2 BACKGROUND Produce will be delivered on an as needed basis, generally two to three times a week. Exact items and quantities ordered are dependent on the weekly menu and the current patient census; however, the most frequent items ordered, and estimated quantities are as follows. Orders will be place via telephone or electronically be authorized Nutrition and Food Service staff. The following positions are permitted to place produce orders: Chief, NFS; Assistant Chief, NFS; Chief of Production, NFS; Supervisors, NFS; and Ingredient Control Clerk, NFS. All produce orders are to be delivered within 24 hours of placement of the order. STATEMENT OF WORK PART B WORK REQUIREMENTS The Vendor: Delivery is to be made to: Wilmington VA medical Center located at 1601 Kirkwood Highway, Wilmington, DE 19805 at an anticipated delivery date between ASAP that will be coordinated between the vendor and WVAMC. For delivery of the purchased produce the WVAMC NFS department excepts deliveries Monday Saturday, 6:00 AM 4:30 PM, STATEMENT OF WORK PART C GENERAL REQUIREMENTS No out-of-date or close-dated items are to be delivered. If the produce is rejected by the NFS representative, it is to be returned by the vendor s agent and a credit is made within one billing cycle. If an item is out of stock, it is expected that once it is received by the vendor, it will be delivered within 24 hours. STATEMENT OF WORK PART D SUPPORTING INFORMATION Place of Performance Wilmington VA Medical Center (hereafter referred to as, The VA or WVAMC) Period of Performance Period covers a base year and option year(s). Base Year - TBD Option Year 1 - TBD Option Year 2 - TBD Option Year 3 - TBD Invoice and Payments: The contractor shall submit an invoice to NFS at the Wilmington VAMC with each delivery of produce. The contractor shall submit a properly prepared invoice for payment. Invoices received without the proper documentation will be returned to the contractor without action and payment will be delayed of disputed until proper invoicing is provided.
