Description
STATEMENT OF WORK Scope: To provide weekly watering and fertilization of existing interior plants at Jesse Brown VA Medical Center (JBVAMC) and Adam Benjamin Jr. Outpatient Clinic (ABJOPC) with a guarantee for all plants. Contractor services to include the rotation of 49 Bromeliad plants 7 times per year at JBVAMC and 6-6 Orchid plants rotated 6 times per year at ABJOPC. (See Plant Inventory Below) Specific Tasks: a. Maintain all interior plants at JBVAMC and ABJOPC that are called out on the list below. Maintenance includes watering, pruning, fertilizing, replacement, guarantee free from pests, and dusting of foliage. Service includes the rotation of 49 bromeliads (7 times a yr.) at JBVAMC and shall provide all food and necessary additives. There is a total of 32 plant containers with 34 non-rotated plants and 49 rotated bromeliads at JBVAMC. Plant total at JBVAMC is 83. Service includes the rotation of 6 orchids (6 times a yr.) at ABJOPC and shall provide all food and necessary additives. There is a total of 39 plant containers with a total of 33 non-rotated plants and 6 rotated orchids at ABJOPC. Plant total at ABJOPC is 39. Contractor shall provide needed service on a weekly basis at both locations. Contractor shall provide a weekly service report stating the date and exact type of service performed. Contractor shall replace a damaged or dying plant, at no additional cost within 2 weeks of its approval and documentation on the weekly service report. If plant material is a special order or is unavailable vendor will replace it with a mutually agreed upon variety. A contractor supervisor shall perform a quarterly on-site review of the account. The contractor shall respond to a call within 24 hrs. Contractor will either return the call or a personal visit will be made. Contractor shall provide blooming plant program assuring that that there will always be a flowering or colorful plant that is exchanged as the bloom fades. Orchids at ABJOPC and bromeliads at JBVAMC. Hours of Operation: Normal hours of coverage are Monday through Friday from 7:30 am to 4:30 pm CST, excluding holidays. All services will be performed during normal hours of coverage unless requested or approved by COR or designee. Holidays include: New Years' Day Labor Day Martin Luther King Day Columbus Day Presidents' Day Veterans' Day Memorial Day Thanksgiving Day Independence Day Christmas Day And any other day specifically designated as a national holiday by the President of the United States. Madison St. Crown Point, IN 46307 Period of Performance: Base Year: 1 September 2018 through 31 August 2019 Option Year One: 1 September 2019 through 31 August 2020 Option Year Two: 1 September 2020 through 31 August 2021 Option Year Three: 1 September 2021 through 31 August 2022 Option Year Four: 1 September 2022 through 31 August 2023 Performance Monitoring: Various methods exist to monitor performance. The COR shall use the surveillance methods listed below in the performance monitoring of this contract. Direct Observation-(Can be performed periodically or through 100% surveillance.) Periodic Inspection-(Evaluates outcomes on a periodic basis. Inspections may be scheduled [Daily, Weekly, Monthly, Quarterly, or annually] or unscheduled, as required.) ACCEPTABLE PERFORMANCE The Government shall document positive performance. Any report may become a part of the supporting documentation for any contractual action. UNACCEPTABLE PERFORMANCE When unacceptable performance occurs, the COR shall inform the contractor. This will normally be in writing unless circumstances necessitate verbal communication. In any case the COR shall document the discussion and place it in the COR file. When the COR determines formal written communication is required, the COR shall prepare a Contract Discrepancy Report (CDR), and present it to the contractor's program manager. The contractor shall acknowledge receipt of the CDR in writing. The CDR will specify if the contractor is required to prepare a corrective action plan to document how the contractor shall correct the unacceptable performance and avoid a recurrence. The CDR will also state how long after receipt the contractor has to present this corrective action plan to the COR. The Government shall review the contractor's corrective action plan to determine acceptability. Any CDRs may become a part of the supporting documentation for any contractual action deemed necessary by the CO.
