Description
3 12/10/2018 This is a SPECIAL NOTICE ANNOUNCEMENT ONLY.   It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract.    Requests for a solicitation will not receive a response. Responses to this Sources Sought must be in writing.   The purpose of this Special Notice Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HubZone and other Small Businesses interested and capable of performing the work. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition.      Responses to this notice shall include the following: (a) company name b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of small business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUBZone, Women Owned Small Business, Small disadvantaged business, or Small Business HUBZone business and (i) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. The Department of Veteran Affairs NCO2 would like to acquire an Anesthesiology Service contract for the New York Harbor Site. Stratton VA Medical Center Albany, New York Statement of Work (SOW) VHA Customer Reference Guide Appendix A: Statement of Work Writing Guide VHA Business Intelligence Tool (VABIT) - Contains sample PWS, Salient Characteristics, Market Research Templates, QASP s etc Background: The Stratton VA Medical Center in Albany New York is currently seeking.state what service is being requested. List the equipment manufacturers, model numbers, item types that are pertinent to the requirement. Give a clear, brief (max of 4 sentences) synopsis of why the service is needed. Is it a repair of a broken piece of equipment? Is it a preventative maintenance service? Is it an inspection? Is it an installation? Is it a routine test or some other type of service? Scope of Work: Contractor is required to furnish all parts and labor to accomplish the task of state the full scope of the task 1. If parts are required, do they need to be original equipment manufacturer (OEM) parts, in accordance with OEM guidelines? Contractor is required to furnish all parts and labor to accomplish the task of state the full scope of task 2. If parts are required, do they need to be original equipment manufacturer (OEM) parts, in accordance with OEM guidelines? Continue this for all tasks to be performed. Contractor shall test the functionality of the equipment upon completion of repair. If this is a test for repair, the Test should ensure that damaged parts have been successfully repaired and all functions including, but not limited to those related to repaired parts, are operational. If this is a test for a routine PM service, inspection, etc., then the test should just ensure that the equipment is functioning within the guidelines of the OEM or the applicable standards (OSHA, ASSE, JCAHO, NEFP, etc). Are there any deliverables that need to be supplied such as a certificate of completion or inspection? Report of findings? Finished end products? If technician must be authorized by the OEM or some certifying body then that must be stated in the SOW as well. Safety Requirements In performance of this contract, are there any safety requirements or federal, state and local requirements that the contractor must adhere to? Are there any licenses or certificates to be provided to prove technician s proficiency? The following statement is a good template to address non-compliance. The Contracting Officer or his/her designee will notify the contractor of any noncompliance with the foregoing provisions and the action to be taken. The Contractor shall, after receipt of such notice, immediately correct the conditions to which attention has been directed. Such notice, when served on the Contractor or his/her representative at the site of the work, shall be deemed sufficient for the purpose aforesaid. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all and any part of the work, and hold the Contractor in default. Period of Performance The services POP and hours to be performed must be detailed in the SOW. If a schedule is required for coordination then that also has to be clearly stated in both the SOW and the Evaluation factors. If it is not communicated that this is vital to the requirement then we cannot find a contractor not technically acceptable for not submitting it. If there are performance requirements such as, inspection should occur quarterly, delivery to occur weekly, all services are to be performed in the first quarter of each option year, delivery as needed, etc. they must be detailed in the SOW. Place of performance Site for the work to be performed must be clearly detailed. If there are multiple sites for multiple tasks then there Inspection If services do not include equipment, then an inspection shall be done by the VA COR/POC upon completion. (Window washing, Janitorial, tree cutting, snow removal, waste removal The COR/POC will perform the inspection in the presence of the contractor personnel, prior to the contractor personnel leaving the facility. No invoices will be processed for payment prior to the completion of POC inspection. If the service is routine and/or performance based then inspections may not occur for every service date but may occur periodically as part of a Quality Assurance Surveillance Plan (QASP). If this is the case then a QASP will need to be provided as a separate document. If a site visit is thought to be in the best interest of ensuring a full understanding of the requirement, then a proposed date should accompany the SOW. This date may change based upon the timeliness of receipt of a completed package. Drawings, and Other Attachments Any additional documents that are to accompany the SOW such as drawings, charts, VA regulations should be detailed in the SOW. This will ensure that the contractors will recognize that there are drawings, blueprints, pictures, etc that should be available to them as part of the solicitation. Government Furnished Property If any government furnished property is required to complete the service then detail it in this section. Otherwise, delete this section or mark it as N/A. Other Related Services (Not Included in the Scope of Work) Any services not expressly stated in this statement of work are not authorized. Services such as inspection, Original Equipment Manufacturer (OEM) required maintenance, or other maintenance and repair vital to the continued functioning of the systems may be added only after written modification from the contracting officer. No other person, including the COR or any other VA personnel are authorized to request additional work be performed by the contractor. Any additional services performed that are not authorized by the contracting officer will be considered inclusive. Aside from the SOW technical evaluation factors may be provided. These are go/no-go items that are required to accurately evaluate a contractor s proposal. Omitting these items or details could result in a contractor being found not technically acceptable or non-responsive. Important Information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this Special Notice Announcement request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach.  The Government will in no way be bound to this information if any solicitation is issued. Currently a total set-aside for Small Business firms is anticipated. However, if response by Service Disabled Veteran Owned Small Business firms proves inadequate, an alternate set-aside or full and open competition may be determined. No sub-contracting opportunity is anticipated. The North American Classification System (NAICS) code for this acquisition is 811219 (Other Electronic & Precision Equipment Repair and Maintenance). Notice to Potential Offerors: All Offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM located on the web at www.sam.gov).  It is desirable that any Offeror to have completed their business Online Representations and Certifications Application (ORCA).    Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov.  All interested Offerors should submit information by email to Clifford.Harrison@va.gov and  should be received no later than 12:00 pm EST on December 17, 2018.
