This request is for informational and planning purposes only and shall not be construed as a solicitation or as an obligation or commitment by the Government at this time. This notice is intended strictly for market research. The purpose of this Sources Sought notice is to determine interest and capability of potential qualifications of Small Businesses, Service Disabled Veteran-Owned Small Businesses, and Veteran-Owned Small Business firms relative to the North American Industry Classification Code (NAICS) 339112 (Surgical and Medical Equipment Manufacturing). The Department of Veteran s Affairs is seeking small business manufacturer or distributors of small business manufacturers, of functionally equivalent equipment to all the following: Items Needed to meet Governments requirement: Brand Name or Equal (BNOE) Clinical Audiometers (Quantity) (Part Number) (Description) #1. (Quantity of 3) (8108091) (BNOE - GSI AudioStar Pro Clinical Audiometer) Includes large color display and built in word list wavefiles. Must perform special tests such as QuickSIN, BKB SIN, and TEN Test. Must be able to function as a standalone unit or PC integrated. Must Include software and cables to operate with GSI Suite or Noah. Must Include transducers: TDH50 headphones, B81 bone oscillator, and IP30 insert phones. #2. (Quantity of 1) (8503145) (BNOE - GSI AudioStar Pro Clinical Audiometer with high frequency option.) Must Include large color display and built in word list wavefiles. Must perform special tests such as QuickSIN, BKB SIN, and TEN Test. Must be able to function as a standalone unit or PC integrated. Must Include software and cables to operate with GSI Suite or Noah. Must Include transducers: TDH50 headphones, B81 bone oscillator, IP30 insert phones and HDA300 high frequency headphones for testing up to 20 kHz. #3. (Quantity of 4) (8108266) (BNOE - Grason Stadler TympStar Pro middle ear analyzer) Must include multiple probe tones frequencies, Must include touch screen interface, and software/hardware for PC connectivity. Printer Optional, Not included. #4. (Quantity of 2) (8070) (BNOE - Standard installation and in-service for four GSI AudioStar Pros and TympStar Pros) Installation includes setup of middle ear analyzer, audiometer and calibration of existing speakers to two audiometers as well as one in-service for end-users. #5. (Quantity of 4) (3547) (BNOE - Audioscan Verifit model VF-2 real ear/hearing aid analyzer) Features include dual probe modules for binaural on-ear measures, test box coupler for performing binaural test box measures, expanded frequency range up to 16 kHz, and a design that allows for efficient testing of total hearing aid performance. Includes two-year warranty and all standard accessories. #6. (Quantity of 4) (VA-201) (BNOE - Audioscan NOAH Module) Must allow for communication between Noah and a Verifit or Axiom so that data from either can be stored, viewed, and/or printed from a PC. Requires the PC and the Audioscan unit to each be connected to the facility's computer network. #7. (Quantity of 1) (8070) (BNOE - Installation and in-service of Verifit 2) Installation of the Noah module software must be installed by VA IT personnel #8 (Quantity of 1) (training) BNO Training for Audiology equipment to all staff) The requester is also asking for the following items to be used as trade-in. Trade in of four GSI 61 audiometers, four TympStar Pro middle ear analyzers, and three Verifit hearing aid analyzers. Four (4) GSI 61 Audiometers (EE#51712, 37717, 51713, 41932). Four (4) TympStar Pro Middle Ear Analyzers (EE#51716, 41933, 51715, 51714). Three (3) Verifit Hearing Aid Analyzers (EE#87694, 48590, 87695). The Contracting Officer will verify that each response is legitimate and verifiable. The Clement J. Zablocki VA Medical Center, located in Milwaukee, Wisconsin, 53295-1000 has a need to acquire Clinical Audiometers for the Audiology Department. The Audiometers are used for hearing tests for all types of exams and for all types of Veteran patients. Middle ear analyzers are used to access middle-ear function to include tympanic membrane movement and acoustic reflex threshold and decay assessments. The Verifits are used to program, assess function of, and troubleshoot hearing aids. The place of performance is Department of Veterans Affairs, Clement J. Zablocki VA Medical Center, 5000 W. National Avenue, Milwaukee, Wisconsin, 53295-1000. ** To prevent Gray Market Items within the VA, the following also applies to this sources sought notice: ** (a) Gray market items are Original Equipment Manufacturers (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers.   No remanufactures or gray market items will be acceptable.  (b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM.   All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. (c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract.   Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.   THIS IS NOT A SOLICITATION AND DOES NOT OBLIGATE THE GOVERNMENT TO ISSUE A SOLICITATION.
