Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is 140A1619A0012 and is issued as an invitation for bids (IFB), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-101. The associated North American Industrial Classification System (NAICS) code for this procurement is 611710 with a small business size standard of $15.00M. This requirement is a [ Small Business ] set-aside and only qualified offerors may submit bids. The solicitation pricing on www.FedBid.com will start on the date this solicitation is posted and will end on 2018-12-12 16:00:00.0 Eastern Time or as otherwise displayed at www.FedBid.com. FOB Destination shall be in the Statement of Work. The Bureau of Indian Affairs requires the following items, Meet or Exceed, to the following: Base Period of Performance: 01/01/2019 - 12/31/2019 LI 001: The purpose of this solicitation is to provide quality, School Psychologist services (diagnostic testing to re-evaluate or determine eligibility for Special Education Services) in the academic setting. School Psychologist services are to be provided to Flandreau Indian School students on an as needed basis in conjunction with the IDEA requirements for testing (60 days from the date of parent consent). The Offeror is expected to provide accurate case notes, assist with program planning, network with internal and external resources, and provide documentation and record keeping tasks necessary to compile reports for funding agencies. The contractor will attend internal and external meetings with Flandreau Indian School staff and/or off campus agencies when necessary for service provision. Specific Tasks to be performed by provider: 1) Provide professional evaluations that are prepared according to the legal definition of IDEA (Individual with Disabilities Education Act). 2) Evaluate and screen students to determine eligibility for Special Education services or students due to have reevaluation. 3) Prepare and provide a comprehensive evaluation report 4) Use the standardized testing instruments supplied by the contractor 5) Generate a schedule by conferencing with the School ™s Special Education Coordinator 6) Provide School Psychologist in-service to the Special Education staff to explain diagnostic summary when necessary or requested by staff 7) Coordinate and maintain record keeping as necessary 8) Comply with all federal mandates, State Board of Education regulations, and BIE policies for service to students with disabilities 9) Services will be provided during school hours up to 16 hours per week 10) Must be available for telephone consultation and willing to adjust professional schedule to meet the students ™ schedules and Special Education deadlines. 11) Participation in regular case consultation 12) Electronic documentation required on date of service 13) Mileage and lodging will not be provided 14) Provide a quarterly report to the Exceptional Education Director detailing services provided including: number of students served in individual sessions, number of referrals of off campus services, and number of referrals to inpatient services. The BIE will approve the assigned psychologist and will notify in writing the school, Education Line Officers, Associate Deputy Director of the region, and parents when the psychologist has been assigned and the name of the individual. The BIE will provide a contact list to all parties, including the contractor, upon assignment of the psychologist.
