***THIS AMENDMENT IS TO NOTIFY POTENTIAL OFFERORS THAT THIS RFQ WAS POSTED UNDER THE WRONG NUMBER AND IS REPOSTED UN 19-233-SOL-00036. THIS NOTICE WILL BE CANCELLED IN IT'S ENTIRETY.*** This RFQ 19-233-SOL-00036 replaces previously cancelled RFQ 18-233-SOL-00606 and is for the Dept. of Health and Human Services, Program Support Center (PSC), Federal Occupational Health (FOH) Wellness and Health Promotion Portal (WHP Portal) This requirement is being conducted as a simplified commercial service conducted under FAR Part 12 and 13.5, as supplemented. The potential award will be a single award IDIQ FFP contract to acquire a web-based, Contractor-Owned/ Contractor- Operated (COCO) Company operated health promotion product that includes comprehensive self-management tools, targeted interventions, and interactive contacts to foster program engagement. It must include online Health Appraisals (HA), (also known as health risk appraisal or HA), personalized healthy lifestyle information, fitness center attendance tracking, and other tailored tracking and assessment tools to deliver a complete interactive health and wellness and health promotion system for eligible Federal employees of FOH's customers. Reference Section L.3 for submission deadlines for questions and proposals.
