Description

REQUEST FOR INFORMATION / SOURCES SOUGHT INQUIRY The Department of Veterans Affairs Network Contracting Office (NCO) 19, Rocky Mountain Acquisition Center (RMAC) is seeking information and sources that can provide Assisted Living Residence (ALR) with psychiatric step-down services in providing the appropriate level of care to veterans discharging from the RMRVAMC inpatient psychiatric mental health care unit (IPU). Place of Performance: The Contractor s facility shall be located within a 60-mile radius of the RMRVAMC, 1700 N. Wheeling Street in Aurora CO, 80045. Qualifications Contractor shall be currently licensed as an Assisted Living Residence (ALR) by the Colorado Department of Public Health and Environment (CDPHE). All services under this contract shall be performed by qualified, licensed staff including: Psychiatrists, Psychologists, Master Social Workers (MSW) Licensed Clinical Social Workers (LCSWs), Licensed Professional Counselors (LPCs), Licensed Marriage & Family Therapists (LMFT), Registered Nurses (RN), Advanced Practice Nurses (APN), Licensed Professional Nurses (LPN), Psychiatric Technicians, Residential Counselors, Physicians Assistants and Pharmacists. Psychiatrists shall be board-certified in psychiatry or board-eligible in psychiatry. Contractor s staff administering medications shall be certified in QMAP (Qualified Medication Administration Person). Services shall be provided daily, or as needed based on clinical considerations and medical necessity. ALL medications and prescriptions shall be issued according to the VA National Formulary unless explicitly authorized by the RMRVAMC via the Bed Flow Clinician. The current VA National Formulary can be found at the following link: http://www.pbm.va.gov/NationalFormulary.aspx Type of Contract: If this request for information leads to a future solicitation, the Government intends to award an Indefinite-Delivery, Indefinite-Quantity (ID/IQ) contract. The performance period shall be one (1) base year with four (4); one (1) option years. This is a request for information and sources only, which may or may not lead to a future solicitation! This is not a request for proposal (RFP). The VA will not pay for any information received resulting from this sources sought notice.
