- Please note, this is NOT a request for quote or proposal. The Government is seeking information strictly for market research purposes only. The Government may or may not issue a solicitation document. The purpose of this sources sought notice is to gain knowledge of potential qualified sources and their size classification relative to NAICS Code 621111 (business size standard $11.0 Million per annum). Responses to this sources sought notice will be used by the Government to makes appropriate acquisition decisions. Reponses should include: company name, service capability statement, examples of similar facilities to which same or similar services were provided, DUNS number, including address, POC, and the size of the business. Contractor shall provide all contractor s physicians necessary to perform 1.0 full-time equivalent (FTE) (or, 2,080 annual work hours) non-personal health care services of board-certified/board-eligible on-site cardiovascular services for the Richard L. Roudebush VA Medical Center (RLR VAMC), 1481 W. 10th Street, Indianapolis, Indiana 46202. A contractor s physician providing on-site cardiovascular services shall meet or exceed the American Board of Thoracic Surgery (ABTS) guidelines. Contractor s physicians (as appropriate and within scope of practice/privileging) shall be responsible for providing cardiovascular care, including the following: performing cardiac surgeries, including pre-operative and post-operative care; charting, writing, and signing notes, such as intraoperative cardiac operative notes, admission notes, discharge summaries, pre-operative attending notes, inpatient progress notes, consults, and outpatient progress notes and consults. Contractor s physicians shall be on-site no later than 7am local time to facilitate an efficient start of the surgical day. Cardiac Surgery clinics are hold on Thursday and Friday from 8am to 11:30am. The Operating Room (OR) days are Monday, Tuesday, and Wednesday from 8am to 4:30pm. Contractor s physicians shall be present, on-time, and in location for any scheduled clinics/surgeries. Outside of clinic time, the remainder of time is spent in the OR. Contractor s physicians shall be fully committed to VA patients and shall not perform any non-VA responsibilities while providing services at the RLR VAMC. Contractor shall make the contractor s physician available for on-call during all hours when the RLR VAMC cardio-thoracic clinic and OR are closed, including evenings, weekends, and holidays. On-call specific days and times of coverage shall be defined as Monday through Friday, 4:31pm to 7:59am, and Friday at 4:31pm to Monday at 7:59am. Contractor shall provide on-call services on weeknights, weekends, holidays, and emergent cases. The contractor shall be responsible for providing a list of on-call contractor s physicians to the Surgery Service COR or Surgery staff designee, prior to the first day of the pertaining month. The on-call contractor s physicians shall be required to carry a pager and/or cell phone and reply within 15-minutes of receiving the page/call. The on-call contractor s physician shall arrive within 60-minutes of receiving the page/call, if any intervention or operation is required. The on-call contractor s physician shall be physically present at the RLRVAMC as direct result of on-call response for the contractor to receive contribution of FTE hours. Applicable NAICS Code 621111, Offices of Physicians (except Mental Health Specialists), applies with a business size standard of $11.0 Million, and the contractor shall be registered and verified in the Systems for Award Management (SAM). Responses to this sources sought notice need to be submitted in writing and received no later than 4pm EST, Wednesday, November 28, 2018. No telephone inquiries shall be accepted or returned. A solicitation is NOT currently available and may or may not be available in the future. If a solicitation is issued, it will be announced on FBO at a later date, and all interested parties must respond to the solicitation announcement separately from the responses to this announcement.
