Description

To whom it may concern; The Department of Veterans Affairs intends to enter into a purchase agreement with: PETNET Solutions Inc 810 Innovation Drive, Knoxville, TN 37932 PETNET Solutions Inc, is the manufacture and developer of FLORBETAPIR (BRAND NAME AMYVID). The VA Medical Center requires this Medication Amyvid FDG/F18 currently in use by VA Boston. It has rapidly become the standard radiopharmaceutical used to identify plaque in the brain. This plaque has proven to be an effective predictor of dementia, which makes it a vital medication for patient brain scans. The VA is not aware of any comparable product or substitute which can meet our high standard of care, and the need for specialized delivery times. The SOW attached here in states the needs of this requirement. Any interested parties who can identify their capabilities and meet our needs should respond to the requirement and furnish detailed data concerning their capabilities to provide the product. This notice is not a request for competitive quotes; however, any firm that believes it can meet the requirements must provide written notification to the Contracting Officer by November 14th at 1400 hours EST. Information should be sent to Sean Powers, Contracting Officer, at sean.powers2@va.gov Information received will be considered solely in determining whether this Item is a Limited Source. If no responses are received by the response date and time, The Department of Veterans Affairs will proceed with the intended agreement. All questions concerning this requirement must be in writing and submitted via email to sean.powers2@va.gov All contractors are required to be registered in the System for Award Management (www.samgov). Service Disabled and Veteran-Owned Small Businesses (SDVOSB and VOSB) must be registered and certified in www.vetbiz.gov Vendor Information Pages VIP). STATEMENT OF WORK VA Boston Radiology FLORBETAPIR (BRAND NAME AMYVID) Contractor to provide Radiopharmaceutical product as requested by authorized VA Staff, to support the VA Boston Healthcare System (VABHS) at the following Campuses. West Roxbury campus at 1400 VFW Parkway, West Roxbury, MA, 02132. Jamaica Plain campus at 150 South Huntington Ave, Boston, MA 02130. All radiopharmaceutical supplies provided under this contract shall be F.O.B. Destination; fuel surcharges shall not be allowed nor added to the invoice. All radiopharmaceuticals provided shall be USDA and USP approved. Contractor must be licensed by the Nuclear Regulatory Commission and be regularly established in the business of providing radiopharmaceuticals. Offeror must provide copies of licenses and certifications along with quote to the Contracting Officer. DELIVERY ORDERS: Normally orders shall be placed during the workday between the hours of 8AM and 6PM and delivery shall be required prior to scheduled injection time, depending on what is specified when the order is placed. The delivery order shall specify: contract item (s) and quantity, time (s) of delivery and applicable VA order number. Orders can be placed orally by telephone and/or fax transmission. Contractor shall routinely provide one (1) delivery per day. Any additional deliveries will be subject to the emergency delivery fee. As it is necessary to the patient. Failure to perform in accordance with the delivery commitment may be grounds for termination of contract in accordance with the provisions for default.
