Page 1 of 1 This Sources Sought Notice is for informational and planning purposes only and shall not be construed as a solicitation or as an obligation or commitment by the Government. This notice is intended strictly for market research. The purpose of this Sources Sought Notice is to obtain information from qualified vendors with special interest relative to the North American Industry Classification Code (NAICS) 339112, Surgical and Medical Instrument Manufacturing. Requirement The Dayton VA Medical Center has a requirement for low air loss therapy mattresses with air mattress blowers. These mattresses are to be used for patients with all levels of skin alterations as an outpatient treatment for home use to maintain patients skin integrity and promote healing of ulcers/wounds. Additionally, these mattresses must meet the following requirements: Mattresses shall be available in Standard (84 x36 ) and Bariatric (84 x39 ); Mattress shall be weight rated up to 600lbs for Standard and 1000lbs for Bariatric; Mattresses shall provide low air loss therapy; Mattresses shall have alternating pressure therapy and pressure redistribution to treat and prevent pressure ulcers; Mattresses shall keep skin cool and dry with moisture control to promote healing; Mattress shall be made of material that is waterproof and vapor permeable; and Mattresses shall offer individual air cushion and adjustable pressure for patient control. Delivery shall be made within 30 days of award to the Dayton VA Medical Center ATTN: Prosthetics, 4100 West Third Street, Dayton, Ohio 45428-9000. Interested vendors are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the System for Award Management (SAM) database prior to award of a contract. Registration is available at www.sam.gov. To be eligible to respond as an SDVOSB or VOSB, a firm must be Center for Veterans Enterprise (CVE) registered in VetBiz. Registration may be accomplished at http://www.vip.vetbiz.gov. Responses to this Sources Sought Notice must contain the following information: Company name and address, point of contact with phone number, and DUNS number; Business type (SDVOSB/VOSB, SB, WOSB, etc.); Capability statement; Provide product description and pictures; FSS contract number, if applicable; and Authorized distributor letter, if your company is not the manufacturer. Responses shall be sent via electronic mail to Angie Carpenter at angie.carpenter@va.gov with the Subject: RFQ 36C25019Q0133, Air Mattresses and Blowers. The deadline to respond is Tuesday, November 13, 2018, at 4:00PM EST. As a reminder, this is a sources sought announcement only.
