- Floor Plans DOL Wage Rates Questionnaire Solicitation This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Solicitation 12475619Q0003 is being issued as a Request for Quotation (RFQ), to provide janitorial services for the Tellico Ranger District Office, Dwelling and Fitness Room Tellico Plains, TN as a base plus 4 option years. This acquisition is set-aside for small businesses. The North American Industry Classification System (NAICS) code is 561720 and the size standard is $18 Million. Quotes are to be submitted to: Frederick Cheeks, USDA Forest Service, 2946 Chestnut Street, Montgomery, AL. NO FAX QUOTES WILL BE ACCEPTED. E-mail quotes are preferable at fcheeks@fs.fed.us Quotes must include vendor's DUNS number. Vendors must be registered in SAM (www.sam.gov) Quotes are due November 21, 2018 at 4:30 PM Central Time. No Pre-Bid conference will be held. Contractors are encouraged to visit the site before providing quote. Contact James Humbert at 423.253.8400 or Deborah Dalton at 423.253.8400 to schedule a site visit. The following provisions and clauses are hereby incorporated into this RFQ by reference; (full text of the clauses may be accessed electronically at www.arnet.gov/far/): FAR 52.212-1, Instructions to Offerors-Commercial Items (OCT 2018) and FAR 52.212-4, Contract Terms and Conditions-Commercial Items (OCT 2018). The following provisions and clauses are hereby incorporated into this RFQ in full text: FAR 52.212-2, Evaluation- Commercial Items (OCT 2014), FAR 52.212-3, Offerors Representations and Certifications-Commercial Items (OCT 2018) and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes of Executive Orders-Commercial Items (AUG 2018). This procurement requires the use of Biobased products to the extent that such products are reasonably available, meet agency or relevant industry performance standards, and are reasonably priced. The products should first be acquired from the USDA designated products categories. The anticipated contract period is December 1, 2018 through November 30, 2019, with four (4) option years.
