- Purpose and Objectives: The intent of this Sources Sought Notice is to identify potential offerors capable of providing ARTHREX Trimano Fortis Shoulder Device and Adapter. These are for the Lebanon VA Medical Center in Lebanon, PA 17042. Responses to this Sources Sought Notice should demonstrate the firm's ability, capability, and responsibility to provide the principal components of supplies listed in the attached document. Responses should include the following information: Verify that the business is under 339112 of the North American Industry Classification System (NAICS), Verification of Size Standard under the NAICS, Business name, address, Point of Contact, Federal Supply Schedule, and if the business has verification as a Service Disabled Veteran Owned Small Business or Veteran Owned Small Business. All information is to be submitted via e-mail at Daniel.Johnson44@va.gov. Information provided will not be returned. All responses shall be in the English Language. Due to the time sensitive nature of this procurement, Responses are due by 2:00 P.M. (EST) on Friday, October 19, 2018. No submissions will be accepted after this date and time. Questions can be submitted electronically to Daniel.Johnson44@va.gov. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. This notice does not constitute a formal Request for Quote (RFQ), nor is the government obligated to issue an RFQ. In addition, the Government does not intend to pay for any information provided under this notice. The Government is not obligated to notify respondents of the results of this survey. Arthrex Trimano Shoulder Support Device Statement of Need Background: The Lebanon VA Medical Center (LVAMC) has a requirement for two (2) Arthrex Trimano Shoulder Support devices, and five (5) Trimano Fortis Adapters. The devices we currently utilize are inoperable. One is unable to be repaired and the other is out for repair at the present time, leaving us with no working device. Scope of Work: This device is used by Orthopedic Surgeons to correctly, and precisely position the shoulder in correct alignment during surgery. The devices purchased must be Arthrex, as we already have the disposable sterile sleeves and additional accessories used with this device in stock. Also, the Orthopedic staff is trained to use this device, and would not need to have additional training in order to perform the shoulder surgery cases. The adaptors are the only coupling devices that will work with the Arthrex Trimano Shoulder support device, and are needed as they are the intermediate element to couple the sterile set (Arm Holder, Wrap, and Sterile Cover). Salient Characteristics of the Arthrex Trimano Shoulder Support Device: Affixes to the Operating Table Rail One hand release Weight Support Arm Approximately 3.4kg Weight Quick Change Coupling Approximately 0.6kg The Fortis Adaptor (Coupling Device) fits onto the Retaining Arm Device and engages into and locks into the arm securing the sterile set. This Arthrex Trimano Fortis Adaptor is the only one which will work with the Trimano Support Device. Additional Information: The Arthrex Trimano Should Support Device is the only device that will work with the Arthrex accessories we currently have in stock at Lebanon, and this is also the unit the OR staff and Surgeons are trained to use. Delivery: Delivery shall be within thirty (30) days from the time of award. The Arthrex Trimano Shoulder Support Device shall be delivered to the Lebanon VAMC located at 1700 S. Lincoln Ave., Lebanon, PA 17042; Attn; Warehouse. BioMedical Engineering will need to perform a safety check and enter the equipment into the system for annual Preventive Maintenance (PM) checks. Placement in the LVAMC clinic shall be coordinated through the OR Nurse Manager and/or Contracting Officer Representative.
