Sources Sought Notice for Ambulance Service at VAMC Cincinnati | Seeking qualified Small Business prospective offerors with OMTB certification to determine set-aside type for the forthcoming solicitation The purpose of this sources sought notice is to seek responses from all qualified Small Business prospective offerors to include SDVOSB/VOSB verified vendors who hold a current Ohio Medical Transportation Board (OMTB) certificate to perform ambulance service in the state of Ohio to determine the type of set-aide that will be utilized for the forthcoming solicitation, summarized as follows: Short Title: Ambulance Service (Advanced and Basic Life Support) for VAMC Cincinnati NAICS-621920: Ambulance Services Purpose: The purpose of this sources sought notice is to determine what type of set-aside is appropriate for this acquisition Place of Performance: Department of Veterans Affairs | Cincinnati VA Medical Center (VAMC), 3200 Vine Street, Cincinnati, OH 45220 Anticipated Period of Performance: 22 January 2019 to 21 January 2024 (Base Year plus Four Option Years) General Requirements: The contractor shall provide all vehicles, personnel, management, transportation, materials, supplies, and equipment necessary to perform contract services as defined and specified in the PWS which shall be provided in the forthcoming solicitation. The contractor shall perform services under this contract in strict adherence to all current laws and regulations applicable to the state of Ohio unless supplemented to exceed those requirements as specified in the PWS. All certification, equipment, staffing and recordkeeping should be in compliance with the rules of the Ohio Medical Transportation Board (formerly the Ohio Ambulance Licensing Board) and appropriate parts of Ohio Administrative Code 4766-3 for ambulette service. Anticipated Requirements: The following are the anticipated not-to-exceed (NTE) quantities for this requirement per year: BLS Ambulance | BASE RATE | One-Way Trip INSIDE Hamilton County: Base rate for all trips with patient transport to, from and within OHIO and INDIANA.BASE YEAR 2,250 = Anticipated one-way trips per year ALS Ambulance | BASE RATE | One-Way Trip INSIDE Hamilton County: Base rate for all trips with patient transport to, from and within OHIO and INDIANA. 630 = Anticipated one-way trips per year BLS Ambulance | MILEAGE RATE | OUTSIDE Hamilton County: Mileage rate for all trips with patient transport to, from and within OHIO and INDIANA outside Hamilton County. 33,750 = Anticipated miles per year ALS Ambulance | MILEAGE RATE | OUTSIDE Hamilton County: Mileage rate for all trips with patient transport to, from and within OHIO and INDIANA outside Hamilton County. 15,750 = Anticipated miles per year BLS Ambulance | WAIT TIME | 15-minute Increments: Wait time of 15-minute increments for all trips with patient transport to, from and within OHIO and INDIANA. 16 = Anticipated 15-minute wait-time increments per year ALS Ambulance | WAIT TIME | 15-minute Increments: Wait time of 15-minute increments for all trips with patient transport to, from and within OHIO and INDIANA. 16 = Anticipated 15-minute wait-time increments per year BLS Ambulance | NO PATIENT | One-Way Trip: No patient one-way rate for all trips with patient transport to, from and within OHIO and INDIANA. 16 = Anticipated one-way no-patient trips per year ALS Ambulance | NO PATIENT | One-Way Trip: No patient one-way rate for all trips with patient transport to, from and within OHIO and INDIANA. 16 = Anticipated one-way no-patient trips per year BLS Ambulance | BASE RATE | One-Way Trip INSIDE Hamilton County: Base rate for all trips with patient transport to, from and within KENTUCKY. 250 = Anticipated one-way trips per year ALS Ambulance | BASE RATE | One-Way Trip INSIDE Hamilton County: Base rate for all trips with patient transport to, from and within KENTUCKY. 70 = Anticipated one-way trips per year BLS Ambulance | MILEAGE RATE | OUTSIDE Hamilton County: Mileage rate for all trips with patient transport to, from and within KENTUCKY outside Hamilton County, Ohio. 3,750 = Anticipated miles per year ALS Ambulance | MILEAGE RATE | OUTSIDE Hamilton County: Mileage rate for all trips with patient transport to, from and within KENTUCKY outside Hamilton County, Ohio. 1,750 = Anticipated miles per year BLS Ambulance | WAIT TIME | 15-minute Increments: Wait time of 15-minute increments for all trips with patient transport to, from and within KENTUCKY. 8 = Anticipated 15-minute wait-time increments per year Ambulance (ALS) | WAIT TIME | 15-minute Increments: Wait time of 15-minute increments for all trips with patient transport to, from and within KENTUCKY. 8 = Anticipated 15-minute wait-time increments per year BLS Ambulance | NO PATIENT | One-Way Trip: No patient one-way rate for all trips with patient transport to, from and within KENTUCKY. 8 = Anticipated one-way no-patient trips per year ALS Ambulance | NO PATIENT | One-Way Trip: No patient one-way rate for all trips with patient transport to, from and within KENTUCKY. 8 = Anticipated one-way no-patient trips per year Attachments for Reference: A draft version of the Performance Work Statement (PWS) is attached for your reference as appropriate. Request all Small Business prospective offerors interested in this requirement email their company name, DUNS number, CAGE code, small business socio-economic size status and a signed copy of their current OMTB certification in Adobe PDF format to perform ambulance service for advanced and basic life support in the state of Ohio for the anticipated subject requirement to bradley.burhite@va.gov not later than 31 October 2018. Only those prospective small business prospective offerors who are certified under NAICS 621920 and submit a current signed copy of their OMTB certification in response to this sources sought notice will be considered responsive for the purposes of this FBO posting.
