- Please complete the RFI Vendor Response Worksheet with indicated information about your Knee Braces. REQUEST FOR INFORMATION (RFI) Knee Braces 1.0 Introduction 1.1 General. The purpose of this request is to obtain information from industry to support the government's intent to establish a Qualified Supplier List (QSL) for the standardization of Knee Braces, to include prophylactic knee braces, rehabilitative knee braces, functional knee braces, unloader knee braces, and patellofemoral knee braces. This Request for information (RFI) is an invitation to submit to the Defense Health Agency (DHA) Medical Logistics (MEDLOG) Division Medical Materiel Enterprise Standardization Office (MMESO) the following: technical information and all related supporting documentation for Knee Braces. This request is a market research effort undertaken by the government which will be used for information gathering purposes. The information received may be used for Acquisition Planning purposes and may help define future requirements for the subject item(s). This is not a request for quote or proposal. 1.2 RFI Submission Package. Vendor must provide responses to this RFI via the RFI Vendor Response Worksheet. Complete answers and all supporting documentation must be provided with the vendor's response. Responses for each salient characteristic should be in the form of yes, no, or alternate proposal. Although explanatory comments may be provided for all responses, they are required for "no" responses. 2.0 Scope Examples of items for Knee Braces product line could include but are not limited to the following distinct categories: 1. Prophylactic Knee Braces designed to prevent or reduce the risk of knee injury for individuals who are engaged in "high risk" activities, such as contact sports; 2. Rehabilitative Knee Braces used during the rehabilitation of a knee joint to protect and control motion following an injury or surgery; 3. Functional Knee Braces designed to provide stability for unstable knees; 4. Unloader Knee Braces designed to decrease compressive forces across the tibiofemoral joint and decrease arthritic pain; and 5. Patellofemoral Knee Braces designed to improve patellar tracking and relieve anterior knee pain. 3.0 Characteristics/Specification Vendors may supply information for one or more Knee Braces categories and associated components and accessories; submission for all Knee Braces is not required. On the RFI Vendor Response Worksheet (attached), please submit detailed responses addressing the below listed characteristics/information concerning Knee Braces as well as providing supporting technical literature and additional options/features/items. Responses must be clear, concise, and specific to the characteristic/information. To facilitate review, vendors must reference applicable document and specific page/paragraph numbers for the specific characteristic/information. Knee Braces 3.1 Are your Knee Braces products manufactured in a Trade Agreement Act (TAA) compliant country? Please identify the country of origin. 3.2 Are your Knee Braces manufactured without natural rubber latex? 3.3 Are your Knee Braces manufactured with breathable, wicking material? 3.4 Are your Knee Braces manufactured with lightweight metal frame? 3.5 Do your Knee Braces come with warranty/return policies for manufacturing or material defects? Provide a copy of the warranty and return policy. 3.6 Do your Knee Braces products come with care and washing instructions? 3.7 Do you provide training material on your Knee Braces products via CD, web-based or manual? 3.8 Could you deliver Knee Braces products and support CONUS (Including Hawaii and Alaska) and OCONUS (Japan, Germany, Korea, Italy, Spain, etc....)? Any information provided in response to this RFI will be considered and treated as unrestricted information which can be used by the Government in any future planning or acquisitions. All information, comments, and suggestions may be incorporated into future acquisition documents, without identifying the source or obtaining prior approval from the source. 5.0 Liability The Government shall not be responsible or liable for any costs incurred by any party in the preparation and submission of any response to this RFI. 6.0 RFI Responses 6.1 Vendor responses to this RFI are to be received by this office no later than 1600 (HST) on 25 October 2018. 6.2 Supporting documentation is required to be provided by the vendor as attachments to this RFI and includes but is not limited to product catalogs, sale sheets, Training Literature, FDA approval documentation and information, etc. 6.3 Responses and/or questions to this RFI should be submitted via e-mail to the points of contact listed below. 6.4 The vendors' responses shall include all detailed information requested in the RFI Vendor Response Worksheet and shall also include: (1) Company name and address; (2) Company POC (Name and Phone Number, and E-mail address); (3) Company website information; and (4) the Company's Commercial and Government Entity (CAGE) Code.
