Page 1 of 102 Page 1 of 102 Sources Sought Notice | Wheelchair/Ambulette Transportation Service for Chillicothe VAMC | SDVOSB/VOSB verified and Small Business prospective offerors with OMTB certification The Department of Veterans Affairs, Veterans Health Administration, Network Contracting Office (NCO 10) is seeking qualified SDVOSB/VOSB verified and Small Business prospective offerors who hold a current Ohio Medical Transportation Board (OMTB) certificate to perform wheelchair/ambulette transportation service (or ambulette service) in the state of Ohio and are interested in submitting an offeror for the following anticipated requirement: Short Title: Wheelchair/Ambulette Transportation Service for Chillicothe VAMC NAICS-485991: Special Needs Transportation Purpose: The purpose of this sources sought notice is to determine what type of set-aside is appropriate for this acquisition (i.e. SDVOSB/VOSB, Small Business, etc.) Place of Performance: Department of Veterans Affairs | Chillicothe VA Medical Center, 17273 State Route 104, Chillicothe, OH 45601 Anticipated Period of Performance: 1 February 2019 to 31 January 2024 | Base Year plus Four Option Years General Requirements: The contractor shall provide all vehicles, personnel, management, transportation, materials, supplies, and equipment necessary to perform contract services as defined and specified in the PWS which shall be provided in the forthcoming solicitation. The contractor shall perform services under this contract in strict adherence to all current laws and regulations applicable to the state of Ohio unless supplemented to exceed those requirements as specified in the PWS. All certification, equipment, staffing and recordkeeping should be in compliance with the rules of the Ohio Medical Transportation Board (formerly the Ohio Ambulance Licensing Board) and appropriate parts of Ohio Administrative Code 4766-3 for ambulette service. Attachments for Reference: A draft version of the Performance Work Statement (PWS) and draft version of the SCHEDULE OF SERVICES PRICE SCHEDULE are attached for your reference as appropriate. Request all SDVOSB/VOSB and Small Business prospective offerors interested in this requirement email their company name, DUNS number, CAGE code and a signed copy of their current OMTB certification in Adobe PDF format to perform wheelchair transportation service (or ambulette service) in Ohio for the anticipated subject requirement to bradley.burhite@va.gov not later than 4 October 2018. Only those prospective offerors who are verified as SDVOSB/VOSB and Small Business certified under NAICS 485991 and submit a current signed copy of their OMTB certification in response to this sources sought synopsis will be considered responsive for the purposes of this FBO posting.
