- RFQ Attachment RFQ Document Purpose: The solicitation is issued to request quotes in support of a Well Drilling project for the Indian Health Service (IHS). IHS is seeking a qualified contractor to construct a new water well, conduct pump test and water sampling, and install a pitless adapter. Option bid items include installation of well pump and connect to existing water supply and electrical power on the Makah Indian Reservation located in Clallam County, WA. Description: The proposed contract listed here is a 100% set-aside for Small Business concerns. Therefore, the Government will only accept offers from certified small business concerns. All other firms are deemed ineligible to submit offers. The Scope of Work for this procurement includes furnishing all tools, equipment, materials, supplies, transportation, labor, and any associated cost with one or more of the following: •1. Drill (1) new community well. Conduct pump test and water testing and install a pitless adapter. Optional: •2. Install well pump, drop pipe, pump cable, level sensor and appurtenances. •3. Connect new well to existing power and water supply line. Disconnect existing well #1. •4. Remove pump and drop pipe from existing well #1. •5. Install new level sensor in existing well #1. All work shall be in accordance to industry standards to include but not limited to: •1. The new well will be fully developed and tested in compliance with SDWA requirements for a new source. •2. Water connections and waterlines must be constructed in accordance with Makah Tribal Water Authority standards. •3. Tribal Regulation(s), as applicable. The performance of the work shall be in Clallam County, WA The estimated magnitude of construction of this requirement per FAR 36.204 is $100,000 and $250,000. The North American Industry Classification System (NAICS) Codes are 237110- Water and Sewer Line and Related Structures Construction, with a Small Business Size Standard of $36.5 million. Competitive Acquisition: The solicitation will utilize Lowest Price Technically Acceptable source selection procedures at FAR 15.101-2. Contract Type: Indian Health Service intends to award one Firm Fixed Price (FFP) Construction Contract in support of this requirement. Period of Performance: The Period of Performance will be for 120 days after the date of Notice-to-Proceed. Respondents are advised that registration in the Systems for Award Management (SAM) database is required prior to award of a contract. Failure to register in the SAM database may render your firm ineligible for award. For more information, check the SAM Website at http://www.sam.gov. Completion of the electronic annual representations and certifications are also mandatory prior to the award of a contract.
