This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. • ii. This solicitation FA488718QST04 is being issued as a Request for Quote (RFQ) for Temporary Lodging Services within 18 miles of Luke AFB and/or Davis Monthan AFB, AZ in accordance with Attachment 1, Performance Work Statement (PWS) dated 23 August 2018. Lodging facility will not be considered for an award if the hotel/motel complex is outside of the 18 driving mile restriction stated in the PWS. The Government intends to solicit and establish a firm fixed-price single or multiple Blanket Purchase Agreements (BPA). The period of performance is expected to be five years, from 1 October 2018 through 30 September 2023. • iii. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-100 effective 22 August 2018. • iv. This RFQ is issued under FAR Part 13 - Simplified Acquisition Procedures. This effort is being procured as a total competitive Small Business Set-aside. The North American Industrial Classification System (NAICS) code is 721110, Hotels (except Casino Hotels) and Motels, with a small business size standard of $32.5M in revenue. A single BPA or multiple BPAs will be established as a result of evaluation of quotes received. • v. Specifics. Services shall include s hort-term lodging facilities which are required to house reservist personnel during Unit Training Assemblies such as the 944th Fighter Wing drill, weekend air shows, base inspections and mission exercises on/near Luke AFB, AZ and/or Davis Monthan AFB, AZ in accordance with all terms, provisions, and conditions defined in the PWS and Site Plan. Dates and room requirements will vary. The contractor shall furnish all facilities, labor, materials, travel, equipment, tools, management and supervision necessary or incidental to ensure that lodging services are performed in a manner that will maintain and provide a satisfactory and safe environment. The hotel must have at least a minimum of a three star rating as established by Forbes or a three diamond as established by AAA. • vi. The contractor shall provide single and double occupancy hotel or motel rooms if and when called for by the Contracting Officer, a designated 944th Fighter Wing (944 FW) lodging representative, or a designated 924 FG representative. Payment for authorized guests will be made by the Air Force Reserve. These calls may be placed from 1 October 2018 through 30 September 2023. • vii. Pursuant to FAR Part 52.212-2 below, i f you are quoting as a third party; third party offeror must provide a written statement from the Owner or General Manager of the hotel/motel that gives you permission to quote for their hotel. This written statement must also include a point of contact (POC) at the hotel that is authorized to make changes in the event of emergencies and the third party cannot be reached; along with a statement that the hotel/motel POC is aware and knowledgeable of the terms and conditions of this RFQ; and the hotel/motel POC understands that only a Contracting Officer or authorized caller can make changes to any order. • viii. The following price list includes line item number(s) and items, and units of measure, (including option(s), if applicable to cover the entire performance period of the BPA. Ensure the line items are included in the quotation provided in response to this solicitation. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. The responsible offerors must be registered in the System for Award Management (SAM) database available at ( https://www.acquisition.gov ). Lack of registration in SAM will qualify contractor as ineligible for award. All responsible sources may submit a quotation in response to this solicitation, which shall be considered. • x. The provision at 52.212-2, Evaluation -- Commercial Items, is applicable. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Contract award will be made to the lowest evaluated price meeting or exceeding the minimum acceptability standards for non-cost factors. Quotes will be evaluated on an "acceptable" or "unacceptable" basis. Technical Capability will be rated on an "acceptable" or "unacceptable" basis. The following factor(s) shall be used to evaluate offers: •a. Price; offeror must provide single and double occupancy nightly rates by completing Attachment 5 - Price List Tabs 1 and 2. •b. Technical capability of the item offered to meet the Government requirement; • i. Lodgin Facility Location: Lodging facility complex must be within the 18 driving mile restriction stated in paragraph 1.1.(h.) of the PWS to be considered "acceptable". Lodging facility complex outside this area will be considered "unacceptable" and not considered for award. • ii. Lodging Facility Rating: Lodging facility must meet or exceed the three star/diamond Forbes or AAA rating as stated in paragraph 1.1 of the PWS to be considered "acceptable". Lodging facility rated below three stars/diamonds will be considered "unacceptable" and not considered for award. • iii. Lodging Facility Approval: The Government will not execute any BPA other than with the facility or venue proposed to provide accommodations. Any offer from a source other than the l odging facility itself must include written evidence that the offeror is currently authorized to submit binding quotes on behalf of the venue whose accommodations and/or facility are being offered to be considered "acceptable". Written evidence must contain a statement signed by the Owner or General Manager of the hotel that gives your company permission to quote for their hotel. This written statement must also include a point of contact (POC) at the lodging facility that is authorized to make changes in the event of emergencies and the third party cannot be reached; along with a statement that the POC is aware and knowledgeable of the terms and conditions of this RFQ; and the POC understands that only a Contracting Officer or Authorized Caller can make changes to any order and only the Contracting Officer can approve changes to the Price List. If a site visit is required the third party and the POC will be notified beforehand. Offerors that fail to provide such evidence shall be deemed "unacceptable" and will not be considered for award. Any offer which requires the Government to execute an agreement that renders a hotel or facility provider as a third party or for lodging/hotel accommodations that cannot be provided at offerors facility shall be deemed technically unacceptable and will not be considered for award. • xi. Provisions at FAR 52.212-3, Alt 1 Offeror Representation and Certifications Commercial Items, Alternative I applies. The offeror shall complete only paragraphs (b) of this provision if the offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) website accessed through http://www.acquisition.gov. • xii.
