- Solicitation document HQ003418R0330. Wage Determination, 2015-5147, Rev. 6, dated 1 AUG 2018. (i) This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is HQ00034-18-R-0330. This solicitation is issued as a Request for Proposals (RFP) (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99. (iv) This solicitation is open to all socioeconomic categories. The NAICS code for this procurement is 813110, Religious Organizations. The small business size standard for this NAICS code is $7,500,000.00. (v) Contract line item numbers (CLINs), quantities, and units of measure are included in the solicitation document and its associated attachments. (vi) The Government requires Protestant Chaplain services for the Armed Forces Retirement Home in Gulfport, MS. (vii) The period of performance is expected begin NLT October 1, 2018. The contract is for a 12-month base period and four 12-month option periods. The place of performance is contained herein. Services are inspected and accepted at the Government facility. (viii) The provision at Federal Acquisition Regulation (FAR) 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix) The provision at FAR 52.212-2, Evaluation -- Commercial Items is included. (x) The clause at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. (xi) The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. (xii) Additional Department of Defense terms and conditions terms and condition for the acquisition of commercial items apply. Additional terms and conditions are attached in the solicitation document Provisions and Clauses. (xiii) This is not a Defense Priorities and Allocations System (DPAS) rated order.
