- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued This a brand Name or equal solicitation / request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. This solicitation is issued as a small business set-aside. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112, with a small business size standard of 1000. The evaluation of offers received in response to the solicitation will be based on lowest price only. The North Texas VA Medical Center is seeking to purchase the Brand Name Temperature Management System. All interested companies shall provide quotation for the following: Line Item 1 HYDRA/22 1 Hydra/22" Iron Intern Automatic Retractor Holder Double Arm w/ 3D Swinger Rail Clamp 3/D Swinger (rail clamp) permits complete versatility of positioning at the site of fixation to the operating table. The 3/D swinger allows the bar to be tilted from side to side 180 degrees. With the Iron Intern ®, a simple adjustment of the 3/D Swinger permits rotation of the long bar up to 360 degrees. Line Item 2 NHLRS3 2 Nathanson Hook Liver Retractors, Set of 3 Sizes Nathanson Hook Liver Retractors is designed specifically to retract the liver during laparoscopic surgery. When required, these hooks hold the liver out of the way for the duration of the surgery. They come in a set of three (small, medium, and large) allowing the surgeon alternative options during the procedures. The Nathanson Hook Liver Retractors are easy to clean and lasts as surgical steel grade instruments. During surgery, they are held by Iron Intern ®. Line Item 3 OBINTRO 3 O'Brien Introducer eCMS Item Line ID 13217238 A set of three O Brien instruments is used with the Lap Band ® System from Inamed. In an effort to simplify the Lap Band gastric banding Procedure, it is designed to introduce the band into the abdominal cavity. Line Item 4 4 ARH/22" Iron Intern Automatic Retractor Holder.Single Arm with 3/D Swinger Rail Clamp Automatic Retractor Holder - ARH 22" Standard Iron Intern ® Single pole with 3/D swinger (U.S Patents). Vertical pole is 22" long. Iron Intern ® is a steady retractor always applying the right pressure, enabling you to position the retractor to your exact requirements. Automatic Retractor Holder - ARH 22" Jumbo Line Item 5 LC1 5 LC1 Greenstein Gripper 2 EA eCMS Item Line ID 13217240 The Nathanson is a mechanical retractor, which mimics the human arm (the Iron Intern ®, Automated Medical Products Corp.), is of much utility in general Bariatric, and transplantation surgery. It has now been adapted for laparoscopic surgery by developing Grippers. These Grippers act as a substitute for the human fingers, which hold laparoscopic instruments. Three different Grippers are available; they either fix laparoscopic instruments directly or fix the trocar through which the laparoscope will pass. Three grippers have been developed. The first, Gripper #1 (LC1), is designed to hold standard cylindrical laparoscopic instruments. It has flat gripping surfaces whose intent is to minimize trauma to the gripped instrument by dispersing the region of compression over as large an area as possible. The two other Grippers hold the trocar through which the laparoscope itself will pass. Gripper #2 (LC2) attaches a disposable trocar, while Gripper#3 (LC3) will hold a reusable metal trocar. Gray Market (a) Gray market items are Original Equipment Manufacturer s (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactures or gray market items will be acceptable. (b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. (c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items. Delivery shall be provided no later than 30 days after receipt of order/award of contract. Specify delivery terms: FOB Destination. The contractor shall deliver all CLINs to 4500 S. The government intends to award a contract as the result of this combined synopsis/solicitation that will include the terms and conditions set forth herein.
