Responders are solely responsible for all expenses associated with responding to this sources sought. Small Business, Service Disabled Veteran Owned, Veteran owned, Women Owned, etc Email Address Phone Number Physical Address Capability statement pertaining to Scope of Work Veterans Wellness Support Services Purpose: The Lebanon VA Medical Center (VAMC), located at 1700 S. Lincoln Ave., Lebanon, PA 17042, has a requirement for fitness and wellness services offered to veterans and other general Department of Veterans Affairs (VA) health improvement initiatives. The VA intends to award a five year contract for these services. Scope or Mission: Services to be provided shall include orientation to the fitness center and pool, fitness classes, lifeguard services, wellness program development, operation of aquatic facilities which includes cleaning, maintenance, and support during seasonal events. Period and Place of Performance: Contracted staff shall be on duty at the fitness center/pool during business hours. Hours of operation for the fitness center/pool are Monday-Friday 5:00am-8:00pm, and Saturday 8:00am-1:00pm. The fitness center and pool are closed on Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Christmas Day, and New Year s Day. Modified holiday hours shall be coordinated with the Contracting Officer s Representative (COR) and observed on Christmas Eve and New Year s Eve. The Offeror shall provide the staffing to fulfill the following roles and duties. All staff shall be CPR certified. A Supervisor/Point of Contact shall have credentials, education, or experience that supports the ability to develop wellness programs. In accordance with VA regulations, a Lifeguard shall be present during hours of operation. He/she shall successfully complete and maintain current credentials and competency as appropriate for the program provided. Acceptable certifications and education programs are: American Red Cross (ARC) Lifeguard Training, ARC s Basic Water Safety Instructor, Basic Water Rescue ARC s CPR and Automated External Defibrillation (AED) for the Professional Rescuer Training courses (or its equivalent from some other nationally recognized agency; e.g., Young Men s Christian Association (YMCA), American Safety and Health Institute, etc.) Background: This requirement is designed to provide health and wellness services to both support and supplement existing programs at the Lebanon VAMC such as the VA MOVE! Weight Management Program, Kinesiotherapy services, aquatic programs, and Lebanon VAMC employee fitness initiatives. Performance Objectives, i. e. required results: Support staff shall be available to complete the following tasks: Maintain statistical report to distinguish attendance between YMCA members, veterans, employees and guests Cleaning of fitness equipment Contractor staff shall be responsible for cleaning equipment in the Fitness Center and Gymnasium. This shall include cleaning all seated and handle surfaces with hospital approved disinfectant per manufacturer s direction. All other surfaces shall be wiped down with a damp cloth and dried. All floors of the Fitness Center shall be vacuumed daily and the mirrors cleaned daily and as needed. The gymnasium floor shall be dry mopped daily and weekly by a floor cleaning machine. The cleaning supplies will be provided by the government. Contractor staff shall be responsible for logging/documenting needs for maintenance of fitness equipment and will contact the COR of such need. Pool Maintenance Contractor staff shall provide general pool maintenance which includes keeping the chemicals balanced and in correct ranges by checking the temperature daily and monitoring the chlorine and PH levels every two hours while the pool is in operation. Contractor shall notify the COR when temperature and/or chemicals are out of compliance. Contractor shall be responsible for general pool maintenance to include vacuuming and shall make available all maintenance and cleaning logs if needed for any VA inspection or review process. The contractor shall notify the COR of any maintenance or operational irregularity. Orientation of gymnasium facilities, as needed Answer questions/provide program assistance for Veterans using the facilities Host fitness classes to include, but not limited to Aqua Fitness, Yoga, Zumba, Bootcamp and Aerobic/Step. Contractor must operate its own fitness facility in Lebanon area that has Gym and pool that is available to VA patients and staff. Contractor must provide membership to VA patients and staff at contractors local Lebanon area Gym so wellness programs can be continued offsite as well as onsite. Any Operating Constraints: Contractor shall provide the COR monthly staff sign-in sheets documenting hours worked. Payments for services and activities will be made monthly in arrears. Contractor shall submit an invoice for the monthly payment and submit to the COR for certification and processing for payment. The Contractor is fully responsible for, and shall indemnify, defend, and hold harmless the Lebanon VAMC with respect to any claims of damage to personal property or personal injury due to the Contractor s gross negligence to any participant while participating in any of the activities. Offerors shall submit a staffing plan that details how the Government requirement will be met. Copies of proposed staff s certifications shall be included with the proposal.
