Description

This is a combined synopsis/solicitation for brand name or equal commercial items prepared in accordance with the format in FAR Subpart 13 as supplemented with additional information included in this notice for the VA Medical Center in Marion, Indiana. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. RFQ 36C25018Q9756 is issued as a request for quotes (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-94, 2005-95/01-19-2017. The North American Industry Classification System (NAICS) number is 333924. The business size is 750 employees. This is a Service Disabled Veteran Owned Small Business Set-aside (SDVOSB). Any contractor that believes they are capable and desires to claim preference as a Service Disabled Veteran Owned Small Business must be registered with the VIP at https://www.vip.vetbiz.gov. Responses shall include: (1) Business Name and Address, (2) FSS Contract Number (3) Vendor Point of Contact Name, Phone Number and E-mail Address (4) DUNS and NAICS code (5) Business Size (6) Type of Business: service disabled veteran owned, veteran owned small business, 8a, HUBZone, woman-owned, etc. (7) If the concern is the manufacture and the sole distributor, the concern shall provide documentation supporting the claim. The offeror shall provide its DUNS, or if applicable it s DUNS+4 number, with its offer which will be used by the contracting officer to verify that the offeror is registered in the SAM database.
