Description
This is a Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources (small and other than small) to perform a potential requirement. Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method. NAICS code: 221310 and sized standard is $27.5M. NOTE: Respondent claiming SDVOSB and VOSB status must be registered and CVE verified in VetBiz Registry https://www.vip.vetbiz.va.gov/.. Project requirements: Location: Memphis VAMC, 1030 Jefferson Ave, Memphis, TN 38104 A. Routine and Preventative Maintenance: Full service and preventative maintenance REPAIR PARTS NEEDED FOR SERVICING MODULAB EQUIPMENT AS FOLLOWS: Mixed Bed Tank, 9" Mixed Bed Tank, 12" Mixed Bed Tank, 10 Cartridge, Micro Post Filters, Filters and Granular Carbon 12 EACH PM (MONTH) AND EMERGENCY VISITS TO CHECK MODULE LAB TO PREVENT BACTERIA FROM GROWING IN WATER. B. Emergency Service: The number of emergency service requests is unlimited. The vendor shall provide 48 hours guaranteed response to a request for emergency on-site service. Monday through Friday, excluding federal holidays. Repairs will be performed between the hours of 8:00 a.m. and 4:00 p.m. Additional time outside specified time may be requested by the VA if determined necessary. C. Parts Replacement: Replacement parts (excluding consumables) are provided with this agreement. Service technicians will carry a stock of commonly used parts for timely instrument repairs. Parts will be of equal quality to the original and meet all manufacturers specifications for said parts. D. Documentation and Certification: Each PM and emergency procedure and inspection shall be documented and certified as having been performed by the signature of the qualified service technician performing the test or procedure. Documentation and certification shall be made available immediately upon completion of repairs of periodic maintenance. Documentation will be provided to the Research Office (Room BC117). Required documentation shall remain on the VA premises and is the property of the VA. E. Quality of Labor: Factory trained and certified service technicians are to perform all service repairs. F. Equipment Availability: The Service Representative will contact the VA, Research SVC to set a mutually agreeable date and time, in advance, for each PM call 4. Delivery. The vendor will provide up to a maximum of two routine maintenance visits per year at 6-month intervals, +/-30 days. Emergency on-site service will be provided, Monday through Friday, excluding federal holidays. Repairs will be performed between the hours of 8:00 a.m. and 5:00 p.m. The VA, Research SVC if determined necessary, may request additional time outside specified time. 5. Progress and Compliance Each PM and Emergency procedure and inspection shall be documented and certified as having been performed by the signature of the qualified service technician performing the test or procedure. Documentation and certification shall be made available immediately upon completion of repairs or periodic maintenance. Anticipated period of performance: October 1, 2018 through September 30, 2019. plus four (4) one-year options. Information sought: Response to this announcement shall not exceed 10 pages and should include the following information: (1) A tailored capability statement indicating the firm s ability to provide a product and related services; (2) The respondents DUNS number, organization name, address, point of contact, and size and type of business (e.g., SDVOSB/VOSB, etc,) pursuant to the applicable NAICS code; (3) Information should be provided electronically in a Microsoft Word or Adobe PDF format.
