Description
The Department of Veterans Affairs, Network Contracting Office 9, intends to award a sole-source, award contract to Medtronic USA, Inc., 6743 S Point Dr. N, Jacksonville, FL 32216 for the Memphis VA Medical Center, Memphis TN. The contract is expected to be awarded under FAR 6.302-1. Period of Performance: Base Year: October 1, 2018 to September 30, 2019 Option year one: October 1, 2019 to September 30, 2020 Option Year two: October 1, 2020 to September 30, 2021 Option Year three: October 1, 2021 to September 30, 2022 Option Year four: October 1, 2022 to September 30, 2023 NAICS code: 811219, SBA Size Standard $20.5 Million. PSC: J065. Medtronic USA, Inc. shall furnish all necessary travel, labor, materials, tools, documentation, and parts required for the inspection, support, and repair of the equipment, upgrades, and accessories for three Midas Rex Consoles with four associated drill trays. PARTS: Medtronic USA, Inc. shall furnish and replace all parts for the equipment on contract. These parts are included in the price of this contract. Medtronic USA, Inc. shall have ready access to all equipment parts, including unique and high mortality replacement parts, in order to minimize downtime and meet equipment uptime requirements. Only new standard parts (manufactured by the maker of the equipment or equal thereto) shall be furnished by the Contractor. Rebuilt parts, used parts, or those removed from the same model of equipment shall not be installed without specific approval by the COR. All parts supplied by the Contractor shall be of current manufacture and have complete versatility with the presently installed equipment. All parts shall perform identically to the original equipment specifications. The services required meet Department of Veterans Affairs' legal obligation to continue to receive, without interruption, the maintenance support services which is vital to Patient Care. THIS NOTICE OF INTENT IS NOT A REQUEST FOR COMPETITIVE QUOTES. However, all responsible sources may submit a capability statement or quotation, which shall be considered by the agency on or before September 25, 2018, 3:00pm Central Time to david.milner@va.gov. A determination not to compete this requirement, based upon responses to this notice, is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. The Department of Veterans Affairs will NOT be responsible for any costs incurred by interested parties in responding to this notice of intent. Only written responses will be considered. All responses from responsible sources will be fully considered. As a result of analyzing responses to this notice of intent, the Government shall determine if a solicitation will be issued. Any prospective contractor must be registered in the System for Award Management (SAM) in order to be eligible for award. Interested parties are encouraged to furnish information by email only with RESPONSE TO INTENT TO SOLE SOURCE MEDTRONIC USA, MIDAS REX CONSOLES in the subject line. All documents submitted will not be returned. All interested parties shall submit clear and convincing documentation demonstrating their capabilities to satisfy the requirements listed above to Contracting Officer, David Milner by email at david.milner@va.gov. The capability documentation should include general information and technical background describing in detail the prospective contractor's capabilities and experience that will demonstrate the ability to support these requirements, including proof of training and certification for performing maintenance and service on the listed equipment and systems.
