- This is a COMBINED SYNOPSIS/SOLICITATION NOTICE for Purchase and Installation of Self-Contained Compactors and Cardboard Baler Responses to this Combined Synopsis/Solicitation must be in writing and submitted on the attached Request for Quotation. This acquisition implements a 100% set aside for Small Business. The NAICS Code is 333318; size standard is 1,000 employees. Responses to this notice shall include the following: (a) company name b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) and must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor and provide the commodities described within the Request For Quotation. Self-Contained Compactors and Baler See Requirements/Specifications below. The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this Combined Synopsis/Solicitation. This notice shall not be construed as a commitment by the Government to award a contract, nor does it restrict the Government to a particular acquisition approach. Notice to Potential Offerors: All Offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM located on the web at www.sam.gov). It is desirable that any Offeror to have completed their business Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Small Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov. Requests for information and resultant responses shall be submitted directly to the contracting officer, Paul Speedling at Paul.Speedling@va.gov. NO TELEPHONE CALLS WILL BE ACCEPTED. All submissions should be received no later than 5:00 pm EST on September 13, 2018. STATEMENT OF WORK VA NYHHS - PURCHASE AND INSTALLATION OF SELF-CONTAINED COMPACTOR & CARDBOARD BALER (ST. ALBANS CAMPUS) INTRODUCTION The Contractor shall be required to accomplish all the tasks and fulfill all the requirements of this SOW as described herein: SUMMARY OF WORK a. PREPARATION Site visits to NYHHS St. Albans Campus to assess the logistics of compactor & baler replacement. Obtain confirming measurements. Provide options on the most beneficial positioning and ease of use of the self-contained compactor and baler to be utilized. Assess any and all electric needs, upgrades, voltage, extensions, etc. b. PURCHASE For St. Albans Main Building Loading Dock - Self-Contained Compactor; Marathon RJ 250SC, 39 Yard Self-contained Compactor with 10 HP, 3 phase motor, ram guide system, full rear door gasket, cycon cylinders, key operated start and remote power unit or similar. For St. Albans Main Building Loading Dock - 420HD 9100 Baler for cardboard For St. Albans Textile Care Facility - Self-Contained Compactor; Marathon RJ 250SC, 39 Yard Self-contained Compactor with 10 HP, 3 phase motor, ram guide system, full rear door gasket, cycon cylinders, key operated start and remote power unit or similar. d. PRE-INSTALLATION Perform necessary (if any) outdoor ground maintenance on which the new compactor unit is to be placed i.e. tracks, platform and/or rails. e. INSTALLATION Perform installations of tracks, fittings and the newly purchased units. f. POST-INSTALLATION Return site visit within 30 days to ensure all aspects of the job are complete and working properly. Service calls to repair and/or adjust operation issues with the installed compactor, tracks and or fittings, shall be at NO COST. Period of Coverage: February 2018 May 2018. EQUIPMENT Contractor Is Responsible for utilizing all equipment at its disposal to perform the removal and installation of items contained herein. VA will not provide nor give any access to any VA owned equipment MAINTENANCE Warranty Period is necessary after installation of the compactors & baler; service calls to repair and/or adjust operational issues must extend at least 6 months after installation. CONTRACTOR S QUALIFICATIONS Contractor is required to have appropriate licensures and insurance to provide these services. SAFETY Contractor's Vehicles, Equipment, Operators, Mechanics, Technicians Must Meet All State, Federal And Local Safety/Maintenance Requirements. TIMELY PERFORMANCE From award of contract to the completion of the job should be no more than 60 days. WORK HOURS Normal workdays are 7:00 A.M. - 4:30 P.M. Monday thru Friday. Due to the nature of the job and the room needed to perform the task; weekday and weekend hours 5am 8pm may be employed to minimize any inconveniences to the normal day to day operations of the medical center. No overtime payments will be authorized. Courtesy hours of early arrival or late departure will be allowed. EMERGENCY SERVICES Emergency calls during and after the installation must responded to Six (6) Hours Of Notification. Only three (3) Occasions Of Failure To Meet Deadline will be tolerated. Unacceptable response will be penalized PRIOR TO CONTRACT STARTUP Site visits to NYHHS St. Albans Campus to assess the logistics of compactor replacements. AFTER CONTRACT STARTUP Return site visit within 30 days to ensure all aspects of the job are complete and working properly. No cost as needed service calls to repair and/or adjust operation issues with the installed compactor, tracks and or fittings. NATIONAL HOLIDAYS: The ten holidays observed by the Federal Government are: New Years Day, Martin Luther King s Birthday, President s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran s Day, Thanksgiving Day and Christmas Day and any other day specifically declared by the President of the United States to be a national holiday. HOLIDAY RESCHEDULING: Generally, when it is necessary to reschedule a service and/or pickup because of a holiday; service and/or pickup must occur on the next business day following the holiday (alternate arrangements can be made with individual sites at their discretion).
