EMPLOYEE BIODATA FORM PAST PERFORMANCE FORM ILLUSTRATIVE BUDGET FORM FORMS - TABLES 1-3 SOLICITATION 72065618R00023 - DESIGN-BUILD OF THREE HIV LABS The United States Agency for International Development (USAID) Mission in Mozambique is seeking proposals to provide and perform construction services to Design-Build Three Health HIV Labs as further described in the attached Request for Proposal (Solicitation/RFP). USAID anticipates the award of a Firm Fixed Price contract(s) for a five (5) month performance period as a result of this Solicitation. USAID anticipates that the total amount for the contract(s) for all 3 lots/provinces resulting from this Solicitation will be between $350,000.00 and $450,000.00, over the five-month period, subject to funding availability. This solicitation provides for multiple awards if determined to be in the best interest of the Government. This procurement will be conducted in accordance with Parts 15 and 36 of the Federal Acquisition Regulation (FAR) (48 CFR Chapter 1). The North American Industry Classification System (NAICS) code for this solicitation is 236220. These services are being procured using other than full and open competition procedures in accordance with the ADS 302.3.4.5 Limiting Competition f. Limiting Competition to Local Entities, dated July 21, 2014 and the authority set forth in AIDAR 706.302-70 (b)(3)(ii). Offerors shall refer to Section L (Instructions for the Proposal Preparation and Submission) of the solicitation for additional information regarding proposal requirements. Failure to comply with the submission date and time will deem any submission unacceptable and it will not be reviewed or evaluated. Section C (Scope of Work) of this solicitation provides details on the activities required to be implemented. Section M (Evaluation Criteria) states the criteria by which proposals will be evaluated. Questions to this solicitation can be submitted to USAID/Mozambique no later than September 18th, 2018 at 05:00 pm Mozambique time, by e-mail to Faizallet Sultan at fsultan@usaid.gov with a cc copy to eluma@usaid.gov. A mandatory pre-proposal conference call will be held prior to the proposal submission due date. Any Offeror interested in submitting a proposal in response to this solicitation should participate in the pre-proposal conference call. The conference call will be conducted via telephone in Portuguese. All interested Offerors must submit a contact phone number and names of participants by e-mail to fsultan@usaid.gov and eluma@usaid.gov no later than September 13th, 2018 at 5:00 pm in order to register for the conference telephone call. Early submission of a phone number to the aforementioned e-mail addresses is highly encouraged as multiple conference calls may be held prior to the proposal submission due date. Phone number submissions received after the deadline may be considered at the discretion of the Contracting Officer. Responses to questions that arise during a conference call will be provided as an amendment to the solicitation so that all interested Offerors may benefit. Offerors are expected to visit and examine the work site and surroundings before submitting a proposal, and obtain at their own expense, all information that may be necessary for preparing the proposal. Below USAID has provided contact information of lab personnel so interested Offerors may schedule and conduct site visits during the solicitation phase.
