- Purpose and Objectives: The intent of this Sources Sought Notice is to identify potential offerors capable of providing BRAND NAME Medtronic Model 5392 temporary pacemaker (dual chamber) and Model 53401 temporary pacemaker (single chamber). These are for the Philadelphia VA Medical Center in Philadelphia, PA 19104. Responses to this Sources Sought Notice should demonstrate the firm's ability, capability, and responsibility to provide the principal components of supplies listed in the attached document. Responses should include the following information: Verify that the business is under 339112 of the North American Industry Classification System (NAICS), Verification of Size Standard under the NAICS, Business name, address, Point of Contact, Federal Supply Schedule, and if the business has verification as a Service Disabled Veteran Owned Small Business or Veteran Owned Small Business. All information is to be submitted via e-mail at Daniel.Johnson44@va.gov. Information provided will not be returned. All responses shall be in the English Language. Responses are due by 10:00 A.M. (EST) on Monday, September 10, 2018. No submissions will be accepted after this date and time. Questions can be submitted electronically to Daniel.Johnson44@va.gov. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. This notice does not constitute a formal Request for Quote (RFQ), nor is the government obligated to issue an RFQ. In addition, the Government does not intend to pay for any information provided under this notice. The Government is not obligated to notify respondents of the results of this survey. Statement of Work Temporary Pacemakers The CMC VAMC has a need to replace two dual chambers and four single chamber temporary pacemakers which are older and at end of life cycle. The temporary pacemaker devices are used in urgent/critical patient care situations where short-term demand (synchronous) or asynchronous pacing is indicated for therapeutic, prophylactic, or diagnostic purposes. Currently, CMC VAMC utilizes the single and dual chamber models manufactured by Medtronic. For staff and physician familiarity/competency purposes as well as patient safety, it is the desire of Cardiology to maintain consistency and minimize change. User Criteria: Easy to configure and operate, ergonomic- small enough to grip with one hand and program with the other; long service life >5 years; ease of use requiring fewer manual device adjustments with no sub-menus; easy to view rate, output and sensitivity settings; and a device that adjusts pacing output voltage in response to changes in lead impedance. Specifically, CMC VAMC has a need for two dual chamber temporary pacemakers with the following functionality: Specifications Modes DDD, DOO, DDI, AAI, AOO, VVI, VOO Basic Pacing Rates 30 200 ppm Upper Rate 80 230 ppm Rapid Atrial Pacing Rates 80 800 ppm Output Amplitude Atrial: 0.1 20 mA Ventricular: 0.1 25 mA Pulse Width Atrial: 1.0 ms Ventricular: 1.5 ms Sensitivity Atrial: 0.4 10 mV Ventricular: 0.8 20 mV A-V Interval Paced A-V (PAV): 50 250 Auto 20 300 Manual Sensed A-V (SAV): 50 250 Refractory Period Atrial: 150 500 ms (PVARP) Atrial Refractory after an Atrial event is equal to the AV interval Ventricular: NA Ventricular Blanking Pace: 200 ms Sense: 120 ms Specifics as it pertains to the four single-chamber temporary pacemaker devices requested: Specifications Modes AAI, AOO, VVI, VOO Basic Pacing Rates 30 200 ppm Rapid Atrial Pacing Rates 80 800 ppm Output Amplitude 0.1 25 mA Pulse Width 1.5 ms Sensitivity 0.4 20 mV Blanking 200 ms +5/-30 ms after pace 120 ms +2/-30 ms after sense Delivery: Philadelphia VA Medical Center 3900 Woodland Avenue, Building 2, 8th Floor Cardiology, Room 8A175 Philadelphia, PA 19104
